Healthy People 2020 Healthy People 2020 is a federal health promotion and disease prevention agenda developed to improve the health. According to its developers, the vision of Healthy People 2020 is to promote “a society in which all people live long, healthy lives.”

Goals As stated on its website, Healthy People 2020 strives to do the following: Identify nationwide health improvement priorities Increase public awareness and understanding of the determinants of health, disease, and disability and the opportunities for progress Provide measurable objectives and goals that are applicable at the national, state, and local levels Engage multiple sectors to take actions to strengthen policies and improve practices that are driven by the best available evidence and knowledge Identify critical research, evaluation, and data collection need

The overarching goals that Healthy People 2020 are the following : Attain high-quality, longer lives free of preventable disease, disability, injury, and premature death Achieve health equity, eliminate disparities, and improve the health of all groups Create social and physical environments that promote good health for all Promote quality of life, healthy development, and healthy behaviors across all life stages

LEADING HEALTH INDICATORS The leading health indicators identified for the next decade include the following : Access to health services Clinical preventive services Environmental quality Injury and violence Maternal, infant, and child health Mental health Nutrition, physical activity, and obesity Oral health Reproductive and sexual health Substance abuse Tobacco

ADDRESSING HEALTH CARE RISK FACTORS Every health care provider should be aware of common health risks for any given population. Significant health disparities exist between different ethnic groups . Although biological and sociological factors contribute to these disparities, most disparities are caused by multifactorial interactions involving genetic variation, sociocultural influences, environmental factors (including availability of healthy and nutritious food), and lifestyle behaviors.

Strategies to address health disparities for minorities include health promotion education, risk factor modification, culturally competent health care delivery, and continued research on factors contributing to racial and ethnic variances in disease and injury. Income and education often go hand in hand as they relate to access to health care information, activities, and programming.

Community Health Assessment aNd Group Evaluation (CHANGE ) Building a Foundation of Knowledge to Prioritize Community Health Needs —An Action 22 is a community-oriented planning guide based on Healthy People 2020 with the purpose of encouraging individuals to participate in achieving the overarching goals of the national initiative . “This action guide provides step-by-step instructions for successfully completing the CHANGE too.

The action steps for the CHANGE plan include the following : Step 1: Identify and assemble a diverse team of 10 to 12 individuals Step 2: Develop team strategy to complete CHANGE as a whole team or divide into subgroups Step 3: Review all CHANGE sectors . Step 4: Gather data from individual sites or locations within each sector Step 5: Review data gathered with the community team Step 6: Enter data Step 7: Review consolidated data to determine areas of improvement Step 8: Build the Community Action Plan by developing and organizing annual objectives that reflect the collected data

HEALTH EDUCATION RESOURCES Health care providers must provide education that identifies risk factors for poor health in target populations and discuss effective strategies that can positively affect the well-being of both that individual and the community at large. Although it is important to emphasize the importance of self responsibility in managing lifestyle behaviors and optimizing wellness through healthy habits, using a team approach to health education and social support can expand access to needed resources for populations at the greatest risk for health disparities. The following topics target common health concerns for at-risk populations

The Physical Therapist’s Role In Promoting Health And Wellness. Major role as preventive practitioners. Role includes identifying risk factors and developing health promotion strategies. Design customized exercises that enhance motor control and improve overall fitness. The Physical Therapist determines an individual’s functional capabilities by examining the requisite motor skills and behaviors needed to perform tasks relevant to that person’s role in society. So we deal with physical capabilities plus psychosocial environment and well being.
