HANDLING OF SPECIMEN PREPARED BY SN EZAWATI & SN SARAH HAZWANA
Introduction Surgical specimen management is a critical function of perioperative nursing. Mismanagement of specimen can have egregious impact on patient’s safety. Specimen errors can have long lasting effect for patient including delays in diagnosis, delays in treatment, incorrect diagnosis, incorrect & unnecessary treatment that can leads to additional surgical procedures, longer hospital stays, increase financial burden, emotional psychologic harm & even death.
Specimen errors are categorized according to when the error occurred. The three phase of specimen handling in which errors take place are the pre analytic phase, analytic phase & post analytic phase.
Identification & Labeling To ensure accurate specimen labeling , the perioperative nurse must first confirm the patient’s identification using two patient’s identifier & ensure that the label have the correct patient’s information. Surgical specimen information should be confirm with surgeon using a write down, read back method.
Specimen label should include : Patient identifier ( e.g : name, ic , mrn ) Date and time the specimen collected (for MAC procedure) Specimen source (name) Type of tissue
Collection, Handling, Transfer, Containment, Preservative & Documentation Perioperative nurses and team members should follow CDC’s universal precaution when receiving or handling specimen. Scrub Nurse : Prepare suitable container to receive specimen from surgeon Handling specimen as little as possible to preserve its integrity Confirmed with surgeon regarding the specimen information
Cover the specimen using septic towel technique Prepare space for specimen marking Pass specimen at the side of instrument table & drop between eye level Use non- cushing instrument to prevent damage of specimen structure & discard the instrument used.
Circulating Nurse : Wear proper PPE Prepare appropriate specimen container (leak & puncture proof container ) – tightly closed lid, round, smooth surface, transparency , new & clean. Reconfirmed with surgeon & scrub nurse the specimen’s information. Label the specimen with confirmed patient’s details & specimen information.
Fixative solution – approved by Food & Drug Administration (FDA), check for expiry date, fixation scale 1:10 & do handle the formalin with proper PPE in adequate ventilation room. Form & label – Check all the patient’s details & all information regarding the specimen. Make sure all information is accurate before send the specimen to laboratory . Document all specimen details in operating nursing notes. Pass over the specimen to recovery nurse completely.
Recovery Nurse : R eceive specimen from circulating nurse with 2 patient’s identifier Document all specimen details in SSSL & specimen book Pass over staff ward.
