Hepatoblastoma Imaging findings

malignant < 4 years high AFP

usually > 10 cm, well-defined heterogeneous hemorrhage, necrosis, cysts Ca ++ in 50% of cases fibrous septae  spoke wheel on US, hypo on T2 hypoenhancing compared to liver on all phases

displaces rather than invades vessels or adjacent structures tumor thrombus – enhancing thrombus dilates vessel CT to evaluate for pulmonary metastases

Remember large heterogeneous hypovascular mass with high AFP in < 4 years age
