HOLTER MONITORING

INTRODUCTION In medicine, a Holter monitor is a type of ambulatory device, a portable device for cardiac monitoring for at least to 24 to 48 hours.

definition A type of portable heart monitor that is a small Electrocardiogram device worn in pouch around the neck or waist. It keeps a record of the heart rhythm, typically over a 24 hour period.

INDICATION Patient suspected of having , Valvular heart disease , CAD,tumors,thrombus ,pericardial effusion. To define cardiac chamber size. Evaluated LV function. Visualized vegetation of endocarditis. Organic heart murmur. cont …………

Evaluation of therapy for arrthymias. Angina. Acute nan-cardiac illness. Determine pacemaker malfunction. Mechanism of rhythm disturbance.

CONTRAINDICATION Left main coronary stenosis. Electrolyte abnormalities. Chronic infectious disease. Advanced or complicated pregnancy. Mental impairment leading to cooperate . Fixed rate pacemaker.

Working mechanism of holter monitoring The Holter monitor is small. Its slightly larger than a deck of playing cards . Several wires are attached to the monitor. The wires or leads connected to electrodes that are placed on the skin on your chest with a gel. The metal electrodes conduct your heart’s activity through the wires and into the Holter monitor, where it’s recorded.

PROCEDURE BEFORE THE PROCEDURE Patient instruction Bath prior test Remove jewelries Shave chest. Loose fitting cloths . DURING THE PROCEDURE Always wear the monitor Record activities in a dairy Avoid things that create inference . Follow instruction.

AFTER PROCEDURE Once you finished procedure ,return the device to the medical office along with the dairy you kept while you wore the holter monitor .

NURSING MANAGEMENT for in patient ,nurses must document patient’s progress notes the time of attachment and the prescribed time to be left. The record box should be check before the procedure. And child as a inpatient have to provide bed side monitoring. Should encourage patient care givers to document the activities of during procedure . Removal of holter monitor after procedure and give to cardiac procedure room. To avoid electric blankets and magnets because signals from such devices may affect recording.

COMPLICATION Risk associated with holter monitoring is very rare Prolonged application electrode patches may cause tissue break down. Itchy skin.

conclusion Holter monitoring is a reasonable diagnostic yield in detecting significant cardiac arrthymias or sympatomatic events , however a period of 24 or 48 hours is a benefit period.
