2. INTRODUCTION Brief Overview Introduction to the patient’s case. S etting the context for the presentation. Demographics Age, Gender, Occupation. Relevant medical and social history.

3. Chief Complaints Clearly state the main symptoms that prompted the patient to seek medical attention.

4. History of Present Illness Onset & Duration- Timeline of Symptoms. (when they started, their progression & any fluctuations. Symptoms description. (Detailed description of primary symptoms & any associated symptoms).

5. Past Medical History Chronic Illness. Surgical history. Medications & Allergies.

6. Physical Examination Findings Examination results Visuals or Diagrams (Include images, charts or diagrams depicting physical examination findings if applicable)
