What is Hazard? Any source or substance that has potential to cause harm to people, environment and infrastructure.

Unsafe Act & Unsafe Condition Unsafe Act - Any behavior or action that can potentially cause harm or injury to oneself or others in the workplace . Ex: - Working without authority,using dangerous equipment,Using wrong equipment and failure to use PPE properly. Unsafe Condition - are conditions that can cause harm to people, environment and property. Ex:- Inadequate guards or barriers,defective tools & equipment or materials, hazardous environmental conditions like dusts,smokes,fumes vapors and poor housekeeping

Unsafe Acts & Conditions

Examples of Unsafe Act Working without proper PPE. Improper lifting techniques Using chemical product that is not labelled or labelled incorrectly Using mobile phones while working Improper use of ladder Ignoring Safety signages Lack of awareness about hazards and control measures of the activity Using damaged tools and tackles

Examples of Unsafe Conditions Oxygen Deficiency Presence of toxic gases Presence of flammable gases Poor Housekeeping Presence of poisonous materials slippery or damaged working floor Unguarded Machinery Improperly guarded machinery Loose electrical connections

Why to Report ? Reporting unsafe acts and conditions fosters a culture of accountability and shared responsibility among all employees. It encourages individuals to take ownership of their own safety and the safety of their colleagues. It helps to identify potential hazards at initial stage The corrective actions can be implemented at earliest.

How to Report? Identify the the scenario and immediately report it to your Safety supervisor/ Site Supervisor.

INCIDENT & ACCIDENT Incidents are often defined as unplanned events, such as near-miss injuries, safety breaches, property damage, or minor bumps or bruises. In most cases, an incident doesn’t result in major harm to a person. An accident is most often defined as a situation, hazard, or event which happens suddenly and leads to serious illness or injury.

Type of Incidents Incidents can be : Near miss Property damage Spills and pollution Environmental harm Work disruption Productivity loss First Aid Case Minor Injuries

Near Miss A near miss is an incident or condition that could have led to injury, illness, or damage, but did not. Near misses are also known as close calls, near accidents, or injury-free events.   All Near misses involve people and some type of action.

Examples of Near Miss Equipments dropped from height but no one was injured. A worker’s safety harness is found to be defective during a pre-work inspection, preventing a potential fall. A gas leak is detected in a boiler room before it can ignite or cause health issues. A conveyor belt starts unexpectedly, but an emergency stop is hit before anyone is caught in the machinery.

Why Report and Investigate Near Miss? Identify the root cause and remove before any accident could occur. Near miss occurs more frequent than accidents hence easy to prevent future accidents. Reporting a near miss helps to establish a continuous safe practice at workplace. Provide valuable information to the employees about the potential hazards. Promotes employee involvement in safety program.
