INDIVIDUAL PRESCRIPTION: PRESCRIPTION : It is a direction given to a professional by physician, dentist or another professional authorised by the law to specify medication, treatment, examination or other form of care to be provided to person or group of persons. PRESCRIPTION may be of types: INDIVIDUAL PRESCRIPTION COLLECTIVE PRESCRIPTION

INDIVIDUAL PRESCRIPTION : Individual prescription concerns only to a person who has undergone a medical assessment by the prescriber . COLLECTIVE PRESCRIPTION : Collective prescription concerns to a group of people or one or more clinical situations. INDIVIDUAL PRESCRIPTION METHODS: Individual drug prescription methods is mainly used in small hospital where the medicine are provided by the hospital on the medicine need of individual patient and his/her prescription. Individual drug prescription methods is generally used by the small and private hospital because of reduce manpower required and desirability for individual service.

Inherent in this system is that possible delay in obtaining the medicine require and increase in cost to the patients ADVANTAGES : All prescription are directly checked by the pharmacist Medication errors could be avoided It facilitates interactions between pharmacist, doctor and nurses Low manpower is required Close control of inventory is possible Advantage over the other distribution system is that every medicine is delivered under care of pharmacist, so very efficient care is provided to all the patients.

DISADVANTAGES : Difficulty of dispensing in absence of pharmacist. This system cannot be used by the big hospital. Medicine cost may be increased.

STANDARDS FOR REALTING THE INDIVIDUAL PRESCRIPTION METHODS : When writing an individual prescription whether for a medication, treatment, examination, laboratory test the prescription must include following information: Prescriber information Patient identification Date written and period of validity Documentation of records and other information.

PRESCRIBER INFORMATION: The prescriber information must include Physician name(Printed, block letters) Permit number Name of Institute or Clinic Mail address Telephone number Signature

PATIENT IDENTIFICATION : patient identification should include: Patient name Date if birth Health insurance number Age Gender Age and gender is included to avoid confusion with the same name persons

DATE WRITTEN AND PERIOD OF VALIDITY: Dqte must be included on prescription Period of validity starts on date of prescription but not when medication dispensed. An individual prescription is valid for a period of 24months/1yr from the date of written unless physician shorter or longer the period of validity. Some drugs have limited period of validity such as BENZODIAZEPINES .

DOCUMENTATION OF RECORD & OTHER INFORMATION: The content of each prescription should be written in patient medical records. A diagonal line must be drawn across unused prescription. The physician may also used the space for writing any drug interactions or contraindications .

REFILLING PRESCRIPTION FOR PATIENT IN INDIVIDUAL PRESCRIPTION METHODS : Refilling prescription may be reorder for the patients by submitting a completed LHS-181 prescription order form. By completing prescription order form LHS-181 it includes space for three times prescription. PRESCRIPTION ORDER FORM: Prescription order form may also be mailed, faxed and telephone to central pharmacy. When prescription order is being mailed to central pharmacy, LHS-181 is completed and prescribers originally signed prescription is attached to LHS-181 form.

When the prescription order is being faxed, prior to faxing the prescription order should have Prescriber sign State pharmacy regulations Initials of person faxing the documents Date that the documents being faxed If the drug is scheduled II to V controlled, prescriber DEA no must be included in prescription order that contains prescriber sign. A white sticker containing prescription number, date dispensed, cost of medicine, no of refills attached to the LHS-181 should be submitted.
