History of presenting illness.. A 60 Year old female was referred from private clinic with the complaints of, Chest pain x 3 days Retrosternal Associated with profuse sweating No history of breathlessness/palpitations Patient was apparently normal before 2 weeks, then patient had complaints of vomiting, loose stools, abdominal pain & yellowish discoloration of urine for which taken native treatment as jaundice
Past History: K/C/O T2DM-not on treatment K/C/O Residual polio weakness of Left LL Not a k/c/o HTN/CAD/CKD/EPILEPSY Vitals: BP-100/60mmHg in Right arm in supine position, PR-96bpm, spO2-98% in RA, CBG-308mg/dl
O/E: pt conscious, obeys commands, afebrile Pallor + No icterus No clubbing No pedal edema No generalised lymphadenopathy Dehydration ++
