1. Definitions: * Handover also known as Endorsement / "Shift Report" is a communication that occurs between two shifts of Nurses whereby the specific purpose is to communicate information about patients under the care of Nurses . * ISBAR – The ISBAR ( I ntroduction- S ituation- B ackground- A ssessment- R ecommendation) technique provides a framework for communication between members of the health care team about a patient's condition.

2. ISBAR Communication will occur in between the staff providing care during the following activity: Change of shift, Coverage during staff breaks, Discussions with other health professionals: Doctors, Respiratory therapy, Physiotherapy…
