1AREA PREPARATION - Slip, Trip, Falling and Cut Injury-Medium -All personnel must be Safety inducted by RAPCO/TR.
-Before starting the activities, TBM should be conducted prior to
start work and work permit signed.
-Conduct housekeeping and eliminate trip/slip hazards from
workplace area.
2SETUP MAN LIFT - Fire/Explosion/Collapse -High -Man lift to be directed to set up area by a competent person.
-Area should be compacted and level as per manufacturer
specification and must be fully barricaded around the area
-Check the overhead obstruction.
-Checklist should be prepare before starting the job.
3MAN LIFT
OPERATION
-Risk of injury due to low
awareness of work environment
-Unauthorized operator
-Falling from height
-No proper access
-Falling of Tools
-High - Operator for this task should be trained and competent to use
the man lift.
-While driving man lift along road ways, etc., the occupant of the
work platform of the man lift, must be kept inside the flat form
safety rail.
-Operator should always allow the flagman signal.
-Clear all obstacle overhead and create ample space for the man
lift when entering the work area.
-Inspection sticker or third party should be available .
-Kept 100% tie off.
-Keep Man Lift away from the edge of excavation.
-Never attach over hanging loads to any parts of machine.
-Barricade the working area around the man lift.
-All tools should be secured by rope tie-off .

- High Wind -High -Man Lift will never be operated in strong or gusty wind, if wind
speed exceed 32km/h, the operation must be stopped.
-Electrical Shock -High -The insulated portion of Man Lift should not be altered in any
manner.
-All electrical parts shall be inspected by competent personnel.
-Don’t operate near the high voltage power line.
4ADDITIONAL HAZARDS -Accumulation of debris
-Tripping Hazards
-Medium -Remove all the debris from the site.
-Maintain good housekeeping standards to keep area safe.
-Materials properly stacked and tripping hazards free.
