Production of Reagents A transfusion services offering basic full- range services should offer production of reagents for the transfusion service: ABO grouping Reagents-By artificial immunization of Donors followed by plasmapheresis, and pooling to recover antibody, Defibrination, complement inactivation, addition of sodium azide, colour coding, sterilization and freezing.
Other Reagents Produced by NBTS Other reagents- anti-D ( By immunization of Rh D negative plasma donors) LISS, NISS, PBS. Anti-Human globulin AHG (rabbit immunization) (also contains anti–complement globulin) Red Cell Panels- are used to identify an unusual or rare antibody in donors or recipient. Are usually Group O Rh D positive cells which have been phenotyped for various antigens against which recipients may raise antibody. Are prepared by national centre and distributed to regional centres. Alternatively the national centre may handle the identification process itself.
Blood processing Donor red cell testing ( Donor red cell immunohematology) Investigation for transfusion transmitted infections HLA and HLA serology Preparation of components, fractionated plasma derivatives and
Cold chain, equipment Cold Room Blood bank refrigerators –Blood and red cells, plasma Domestic type refrigerator- Reagents and antisera Freezers- FFP Refrigerated centrifuge Plasma expressor Platelet agitator Microscope, water baths, incubators, centrifuges
Quality Assurance Standards Main areas requiring attention in quality assurance Clerical procedures in all sections Good manufacturing practise Provision of SOPs Specification for and quality control of reagents Collection storage and transportation of samples and blood donations Reporting system for errors and adverse reactions Automation and computation
Quality control Quality control of Techniques Quality control of equipment- selection and evaluation of equipment, maintenance of full logs for each equipment Quality control of performance-external quality assessment schemes involving simulated clinical samples nationally or regionally to examine range of competence for each centre. QC of blood component preparation QC of tests for TTIs Bio safety in the transfusion laboratory
Kenya NBTS History Organization and administration Services Partners challenges
Organization- Organogram Kenya NBTS is a programme under the Curative and Preventive department of MoH, and Blood Safety Programme Of NASCOP in HIV prevention
Administration-Kenya NBTS
Administration-Kenya NBTS The Secretary to the National Board is the Director of NBTS. The director is trained in transfusion medicine There are in total 7 regional centres The National Centre doubles up as the Regional Centre for Nairobi region Each of the regional centre is headed by a Pathologist (director ) and assisted by a Technical Director responsible for operations. All the Regional directors have training in Transfusion medicine Satellite centres are essentially cold rooms for storage
