LAST OFFICE This is the care given to a dead person shortly after death has been confirmed, to ensure clean body and human dignity. This is done while also preparing the body for viewing by family, transport or burial. It is also known as post mortem care

PURPOSE OF LAST OFFICE To release the body to the relatives with respect and dignity To maintain hygiene

REQUIREMENTS TROLLEY : Small tray with ; soap in a dish, Vaseline, nail cutter, pair of scissors, clean pair gloves, a roll of toilet paper, cotton swabs, 2 artery long forceps, dressing pack, make up for men or women as per relatives'’ choice. Bottom shelf with; a plastic apron, bottle of antiseptic lotion, notification of death, forms, jik, a pair of mortuary sheets, 2 buckets one containing disinfectant lotion.

CONTINUATION OF REQUIREMENTS Deceased burial clothes ; dressing clothes, men-underwear, trouser, shirt, coat, tie or traditional culture. Bed side ; handwashing, screens, bucket for wastes, dirty linen container Kit with name tags

NURSING AND MEDICAL MANAGEMENT Notify the in charge as soon the patient ceases to breath. Physicians certify death, document time of death and action taken. Validates state for organ donation, maintain sensitivity to personal, religious and cultural. Inform the relatives with empathy, counsel them and explain to them the procedure to done. Ask if family wishes to participate in the preparations of the body. Request the relatives to bring deceased clothes.

Put on protective gear (apron, gloves, gown) to prevent cross infection. All equipments, soiled linen should be removed from the bedside. Remove any jewel, necklaces, watches that have been worn by the deceased. Body is placed in supine position with upper limbs on either side straightened, palms facing down. One pillow is placed under the head and shoulder to prevent blood from discoloring the face. The eyelids are closed and held in place for few seconds so that they remain closed. Dentures are usually inserted to help give the face a natural appearance.

Soiled part of the body are washed, however complete bath is not necessary. Absorbent are placed under the buttocks to take of any feces and urine released. Clean gown are placed on the client, hair brushed and combed. The top bed linen are adjusted neatly to cover the client to the shoulder. Cotton swabs are kept in nose and ears.

LAST OFFICE IN PRENATAL DEATH

PRENATAL MORTALITY: is death if a baby in the first 28 days of life including stillbirth. REQUIREMENTS: In addition to the adult last office requirements: -baby clothes -diapers -small artery forceps

NURSING AND MEDICAL MANAGEMENT: Communicate empathetically to the mother and father of the death and cause of death if known. For intra-uterine fetal death the mother and next of kin must be informed of what has happened and the next steps. Request the mother if she would want to visualize the baby especially if 4he baby is abnormal or macerated. Find out if the parents would want to take a photo of their dead baby. Put on protective wear, bathe the baby with warm water and soap and dry it with a towel

Pack all orifices with cotton wool, mouth, nose and anus. Ensure cotton is not visible. Fill the mortuary label with the mother’s names, date, time of birth and dead baby’s sex, state if stillbirth or not, name of ward. Put the label on the baby’s arm. Dress the baby fully and wrap it completely in clothes given by the mother. Inform mother and next of kin where the body is to be kept. Take the body to the mortuary well labelled.

APPLICATIONS/USES OF LAST OFFICE Educational purposes: teaching medical and nursing students about the procedures and ethical considerations involved in post-mortem care. Infection control: to prevent spread of diseases to family members and healthcare workers and prevent contamination to the environment. To provide respect and dignity to the deceased: adhering to specific cultural and religious practices in a manner consistent with the deceased’s beliefs.

CONTINUATION of APPLICATIONS/USES OF LAST OFFICE In preparation for viewing and funerals: for family and friends who wish to see their beloved one for one last time. Organ and tissue donation: get permission of the family & coordinate with organ donation agencies to see organs that need to be recovered for transplantation Completion of Legal and documentation requirements such as death certificates.
