What is diabetes? Diabetes mellitus is a persistent, long-standing disease where utilization of carbohydrates, fats, and proteins by the body is disturbed leading to high blood glucose levels or hyperglycemia This occurs due to: Inability of the body to produce enough insulin (due to decreased functioning of beta cells in pancreas) Inability of the body to use insulin effectively (insulin resistance) Both 1 and 2 Hyperglycaemia is a singular constant feature of all types of diabetes
Types of Diabetes Type 1 diabetes: due to beta cell destruction, usually leading to absolute insulin deficiency Type 2 diabetes: due to a progressive insulin secretory defect on the background of insulin resistance Gestational diabetes mellitus: Diagnosed in the second or third trimester of pregnancy
Modifiable risk factors for type 2 diabetes Dietary factors Abdominal obesity Overweight Physical inactivity
The classical symptoms of diabetes (3 P’s) Polyuria, Polydypsia and Polyphagia Frequent urination (Polyuria) Excessive thirst (Polydypsia) Extreme hunger (Polyphagia)
Other symptoms of diabetes Weight loss Extreme fatigue Frequent infections Slow healing of wounds Tingling/numbness in hands and feet Blurred vision
Why should diabetes be treated? Sustained high blood sugar levels can cause damage to: Eyes Kidneys Nerves Heart Blood vessels

Diagnostic criteria of diabetes ADA 2018 Fasting plasma glucose (FPG) ≥126 mg/ dL OR 2-h plasma glucose ≥200 mg/dL during an OGTT OR A1C ≥6.5% OR Random plasma glucose ≥200 mg/dL
Understanding the diagnostic tests for diabetes HbA1c: Glycosylated haemoglobin Glucose in blood circulation binds irreversibly to haemoglobin A hence called as HbA1c Evaluates average amount of glucose in blood over previous 2-3 months HbA1c
What to remember before diagnostic tests? For HbA1c: No fasting required No smoking, tobacco, exercise Continue regular medications Inform doctor if suffering from any disease of abnormal haemoglobin Similarly for random blood sugar testing, no overnight or 8-h fasting is required
How will you identify high-risk population for diabetes? ~50% of diabetic patients remain undiagnosed and that is why diabetes is called as ‘silent disease’ Various laboratory and clinical tests are used for identification of high-risk patients for diagnosis The Indian Diabetes Risk Score (IDRS) , is one such simple screening tool for prediction of undiagnosed diabetes developed by Dr. Mohan and colleagues at the Madras Diabetes Research Foundation (MDRF), Chennai

Can diabetes be cured? No, but it can be controlled and complications can be prevented Patient can lead a normal life if managed well It has been shown that with bariatric surgery, 72% of patients with type 2 diabetes mellitus have achieved near or complete normalisation of blood sugar levels
Can diabetes be prevented? Yes, diabetes can be delayed and prevented Following simple lifestyle measures will reduce the future risk of diabetes : Controlling weight Get moving (exercise) Healthy diet habits Smoking cessation Alcohol cessation As per clinical studies, medications like metformin are known to prevent diabetes among high-risk populations
