What isLock-out?
(3.3.1)
•Lock-outis a system of locks, which secures
devices that control hazardous energy from
release (main disconnect switches, circuit
breakers, valves andclutches).
•Hazardous energy in plant couldbe:
–Super heatedsteam
–Extremely hot boiler feedwater
–Electrical supplies as well aschemicals
used to treat theboilers

The purpose of a “Lockout/Tagout” procedure is to protect employees
from machines and equipment capable of causing injury due to
unexpected energization, release of stored energy or the start-up of
equipment while an employee is performing maintenance or servicing
equipment.

Consider just a fewexamples:
➢A worker is servicing a press, another worker comes by and starts the
press, and the service worker is caught up in the press, causing an
amputation
➢Workersarerepairingaconnectioninpiping,andsomewhereupthat
samelineanotherworkeropensavalve,sendingfluidsdownthepipe
thatultimatelyspillonandburnthemaintenanceworkers
➢A conveyor jams, a worker reaches in to try to clear the jam, the
conveyor jam is suddenly and unexpectedly freed, and the worker is
crushed as aresult
➢A worker is servicing a machine, and at the same time internal wiring in
the machine shorts, causing the maintenance worker to beshocked

use of a distinguishable dangertag.

•Tag-out refers to the tag to
inform as to whohas
locked it out as well as
some additional information
such as whenit will be
turned back on or
energized.

released while a machine or
equipment is beingserviced.
•The ultimate goal of lock-out/tag-out is

•OSHA is the Occupational Safety & Health
Administration of the USGovernment.
•The OSHA Lock Out Tag Out (LOTO)
standard was developed to safeguard
employees from hazardous energy while
they are performing service on machines
andequipment.
•OSHA requires that before service or
maintenance is performed on machinery or
equipment, the machine or equipment be
turned off and disconnected from the
energy source, and the energy-isolating
must be either locked and taggedout.

equipment being controlled can not be
operated until the tag isremoved

DeviceRequirements
Both Lockout and Tagout devices mustbe:
•Durable –must be able to withstand environment and durationfor
use.
•Standardized –according to either color, shape, size, print,and
format.
•Substantial –to minimize early or accidental re-energizing.Locks
must be substantial to prevent removal. Tags must be attached
securely andunmovable.
•Identifiable –clearly identify
employee who applied thedevice.

Management is responsible for:
•Drafting, periodically reviewing,and
updating the writtenprogram.
•Identifying the employees, machines,
equipment, and processes included in
theprogram.
•Providing the necessary protective
equipment, hardware andappliances.
•Monitoring and measuring conformance
with theprogram.

Supervisors are responsiblefor:
•Distributing protective equipment, hardware, and any
appliance; and ensuring its proper use byemployees.
•Making sure that equipment-specific procedures are
established for the machines, equipment and processes
in their area.
•Making sure that only properly trained employees perform
service or maintenance that requirelockout.
•Making sure that employees under their supervision follow
the established lockout procedures whererequired.
