LOTO DEFINITION, SCOPE, AND PURPOSE
Lock-Out/Tag-Out is a safety
procedure that utilizes locks and
tags to prevent the removal of
energy-isolating devices and the
unexpected re-energizing of
equipment and/or processes.Download HSE Docs hseprof.com

9
Classification: Internal Use
PURPOSE OF LOCK-OUT/TAG-OUT
•To ensure all sources of energy that may impact the safe
implementation of a task(s) are isolated and controlled:
•Isolation points are correctly identified
•Isolation follows a structured process
•Isolation points are maintained in a locked-out state
•Control of these locks rests with the LOTO Coordinator
•So that the task can be completed safely as a result of
adequate isolation of all sources of energy and
process fluids.Download HSE Docs hseprof.com

ENERGIZE AND DE-ENERGIZE :
Confirm minimum required PPE’s for low voltage switching as
per Arc flash study
Only Authorized Electrical Personenergizeand de-energize
(switching) for LOTO purpose On MCC\SGor other electrical
systems.
Confirm the proper motor tag number in Substation ; Some
motor tag may cause confusing !

•Covered by work permit
•Measurement purposes in various
instruments
•The source can normally be withdrawn
into a shroud or housing in the
