Quality measurement in health care is the process of using data to evaluate the performance of health plans and health care providers against recognized standards Measurement: Key Performance Indicators (KPIs) They are telling you something about what happened to the patient (outcome)? Or they are telling you something about the process of care? Key Performance Indicators (KPI) is used to measure the organization’s performance over time. A quality measure (KPI) is a tool for making “good decisions”. At measurement phase: you Just Know about the process/outcome Quality Measurement You can not manage what you can not measure

Diabetes KPIs (best practices/standards): Hemoglobin A1C is less than 7 Cholesterol level is less than 200 You reviewed 1000 patient files: Hemoglobin A1C is less than 7 (40% of the patients) Cholesterol level is less than 200 (30% of the patients) You Just Know about the process/outcome: you just find an opportunity for improvement ARE WE DONE AT THIS STAGE? DID WE IMPROVE SOMETHING? NO

F- F ind an opportunity to improve P- Plan the project, assign tasks O- O rganize a team D- Do the works that’s needed C- C larify the current process C- Check the results, and measure changes U- U nderstand the problems/variations in the process A- Act to maintain the changes S- S elect the desired outcome(s)   F .O.C.U.S PDCA F ind an improvement project (initiative): Review related standards & documents Analysis of collected data Identify problems & desired outcomes After you reviewed the 1000 patient files you are still in the F stage of improvement cycle

IS NOT… yelling at people to work harder, faster, or safer Protocols or policies and then failing to monitor their use or effect research just an initial measurement of healthcare processes and outcomes Quality Improvement Main Principle: Improvement Requires Change Change = not just doing something different, but engineering something different at least one step in at least one process

Quality Improvement = Improvement Project Quality Improvement: is a process for closing the gap between actual performance and the desirable outcome (diabetes examples) Improvement Project: It is a concentrated effort on a particular problem in one area of the facility. It will be initiated when measurement data (KPIs) reveal a gap between expected and actual performance.
