PRINCIPLES OF MENTAL HEALTH NURSING
Principle is rules or law that guides one’s action, it is a code or standard that helps people to govern their conduct. The principles of psychiatric nursing provide general guidelines that should be use in nursing care of mentally ill person.
There are general principles that applied to care of psychiatric patient. These principles applied to all psychiatric patient regardless of diagnosis or type of behavior.
The nurse needs to accept the client as a human being who has values and nurse worth. Acceptance is important in nursing because you will care for many people who are different, but they must be accepted. Accept The Patient Exactly As A Human Being
Acceptance does not mean acceptance of inappropriate behavior but accept the person as worthy. The nurse must set boundaries for behavior is unacceptable. Acceptance of the patient is an active process.
Nurses can show acceptance by The nurse spend more time with the client listening to him, hearing what he is saying and understand the meaning of what is said.
Be nonjudgmental and nonpunitive-nurse should not judge patient’s behavior as right or wrong, good or bad.
All direct or indirect method of punishing must be avoided, the patient is not punished for undesirable behavior
Listening to patient another means of acceptance. Listening is more than keeping quite, it includes hearing what he is saying and understand the meaning of what is said.
Permitting the patient to express the emotions. The patient have negative emotion and need to express these emotions without psychological or physical danger to themselves and other, and these emotions are in need of outlet. In fact the ability of the patient to express a negative emotion is often a very healthy sign
Self understanding leads to understanding of others behavior and to be aware of why he behaves as he does, and help to understand the behavior of the patient with whom he works. This will make him able to handle the patient in a good way and give the right nursing care. UNDERSTANDING
It means respect the patient as an individual, as human being who possesses worth and dignity. The nurse should have a basic respect for the patient simply as a person. Respect is increased with understanding of the patient’s uniqueness. RESPECT
Sitting silently with the patient who is crying. Laughing with the patient in response to particular event. Apologizing for hurt. Listening to the patient. Call patient by name. Respect can be seen in different ways:
Patients with psychiatric illness need constant reassurance as they are insecure and uncertain. They have to be helped to develop trust and confidence. REASSURANCE (security)
You will get well. All will end well. Nothing to worry. Your fear is groundless. Nurse must avoid giving false reassurance such as:
Pay attention to the matters that are important to patient. Be aware and accept how the patient really feels. Sit beside the patient even when he does not want to talk, accepting patient’s silence, and the physical presence can be very reassuring the patient How to give Reassurance
Listen to personal problems without showing surprise or disapproval. Provide patient acceptable outlets of anxiety.
Observation of mentally ill patients is directed toward the way of his/her behavior: Everything the patient does or says is observed by the nurse and analyzed to know and understand the motivation behind it. OBSERVATION
Patients must be treated on individual basis because individualized care is the basic right of the any patient. Never label the patient with the disease hence nurse has to avoid using phrases such as mental patient, psychiatric patient or schizophrenic patient, instead use phrases such as patient with mental illness or psychiatric illness or patient with schizophrenia Nursing care should be centered on patient as a person and not on the symptoms
Every procedure should be explained at the patient level of understanding. Explaining the procedure to the patient will reduce his/her anxiety. Procedures should be explained to the patient
Anxiety is the problem with which the patient has been unable to cope. The nurse should carefully study situations that may increase the patient’s anxiety. Unnecessary increase of patient’s anxiety should be avoided
Carrying out the procedure quickly, firmly and efficiently with adequate help. No verbal comments should be made during the procedure. Nursing team must have self-control. Verbal and physical force must be avoided
Nurse must treat everyone with dignity and humanity, they should understand their individual’s needs, show compassion and sensitivity, and provide care in a way that respect all people equally. Care with dignity and humanity
Nurses take responsibility for the care they provide and answer for their own judgments and actions Nurses manage risk, are vigilant about risk, and help to keep everyone safe in the places they receive health care. Accountability and safety
Nurse provide and promote care that puts people at the centered, involves patients, their families in service users, their families in decisions and helps them make informed choices about their treatment and care. Patient centered and self determination
Nurses assess record and report on treatment and must handle information sensitively and confidentially. CONFIDENTIALITY
Altruism is concern for the welfare of others. Nurse must be passionate and sincere to her professional duties. Altruism and Genuineness
It involves sensitivity to the patient’s current feelings and the verbal ability to communication this understanding in a language attuned to the patient Empathetic Understanding
It involves using specific terminology rather than abstraction when discussing the patient’s feeling, experiences and behavior. Immediacy involves sensitivity to the patient’s feelings and a willingness to deal with these feelings rather than ignore them. CONCRETENESS AND IMMEDIACY

Ethics are important in nursing practices as nurses directly dealing with human being who have values, rights and different culture. Nurse must be professionally competent responsible and accountable with normal obligations. Nurse needs to provide nursing services even if it is in conflict with their own beliefs and values.
The code of ethics reflects common values regarding nurse patient relationship and responsibilities and serves as a frame of reference for all nurses in their judgments about patient welfare and social responsibilities. Ethical components of the nurse client relationship
A nurse is primary or secondary care provider, these components are always present
Nurse must have professional intimacy as a care provider. It may be relate physical activities, such as bathing psychological, spiritual and social intimacy that helps in patient’s recovery. Professional intimacy
It is an unconditional positive regard to the patients and her behavior towards client shout not depend on patient’s behavior. RESPECT
There must be fairness (equality) in all the decisions taken by the nurse or health team. Care must be provided without any discrimination based on religion, cast, gender, economic status, etc. justice
Tryst is most important in the nurse-patient relationship as the patient is in a crisis or vulnerable. Initially, trust of patient in relationship is unstable, so it is important for a nurse to be consistent and remain unconditional to the patient. trust
It is the ability of nurse to accurately understand the person’s current feelings and their meanings, and to communicate this understanding to the patient empathy
The nurse has more authority and influence in the therapeutic relationship. The appropriate use of power, in a caring manner, enables the nurse to implement the concept of patient as a partner in meeting his needs. power
The nurse respects the uniqueness of individual in provision of care. The nurse respect the rights of individuals as partner in care and help in making informed. Nurse maintains competency in order to render quality nursing care. The nurse if obliged to practice with in the framework of ethical, professional and legal boundaries. Nurse is obliged to work harmoniously with members of health team. Nurse commits to reciprocate the trust invested in nursing profession by society.
