POSITION THE PATIENT Definition : Setting position the patient on the bed for the comfort or having procedure easier. Goals : To prevent muscle pain To decrease the pressure To make comfort for the patient To have procedure easier
THE KIND OF POSITION Supine Position Position where Someone is lying on his or her back 2 . Prone Position Position where someone is lying face down or on his/her stomach
3. Side lying Position or Sim Position The patient is lying on the left side and chest, the right knee and thigh are drawn up, the left arm is along the back. Goals : To have a rectum examining To have an easier injection To decrease pressure on his or her back.
4 . Fowler Position Position in which the head of the patient’s bed is raised 75 – 90 degrees above the level, with the knees sometimes are also elevated. 5 . Semi Fowler Position Position is similar to fowler’s position but with the head is less elevated, 15 – 60 degrees.
6 . Dorsal Recumbent Position Position of patient on the back with lower limbs are flexed and rotated outward. Lithotomy Position The patient is on supine with hips and knees are flexed and thighs are abducted and externally rotated
8 . Trendelenburg Position The patient is laid flat on the supine position with the legs are higher than the head by 15 – 30 degrees.
Procedures of Positioning The Patient Supine position to Side lying Position Material : Pillows Procedures : Inform Consent Washing hand Prepare the environment Check the patient condition Move the patient’s body and leg to get close to us
6. Move the left hand to the right side of the body 7. Bend the left leg
8. Push together the left parts of the body to the right side 9. Put the pillow on the back of the patient’s body
10. Put the pillow on the right side of the body and ask the patient to hug the pillow 11 . Put the pillow under the left leg, make sure the patient feels comfortable 12. Ask the patient if she feels comfortable.
Remove The Patient from Bed to Wheel Chair Materials : Wheel Chair in locked position Foot wear, such as : sandals, shoes
Procedures : Inform consent Washing hand Prepare the environment Check the patient condition Put the wheel chair on the same level with the bed and set up the foot buffer of the wheel chair. Help patient on the sitting position.
7. Ask patient to take a deep breath, help to make some mobilization, such as : shoulder, foot, toes, and sit for about 1 – 2 minutes. 8. Help patient to use foot wear and lay down foot on the floor
9. Hold patient through axial, ask patient to put her hands on the nurse shoulder (do not put them around nurse neck) 10. Ask patient to stand up, get patient on stable by positioning the patient’s knee against nurse’s, if necessary
11. Point patient to wheel chair and suggest to hold the side of wheel chair 12. Get buckling nurse’s knee while helping patient to sit on the wheel chair air
13. Lay down the patient’s foot on the foot wear of wheel chair 14. Ask the patient if she feels comfortable 15. Take the patient to next examination
