Gout ชายแก่ หญิงหมด mens ดื่มเหล้า เป็นมากที่ 1 st MTP ( Podagra ) และข้อล่างๆ CPPD คนแก่ๆทั่วไป ถ้าอายุน้อยอย่าลืมหา cause มักเป็นตามข้อใหญ่ๆเท่านั้น เช่น เข่า ข้อมือ ผลึก Needle shaped, negative birefringent ผลึก Rhomboid Shape, Positive birefringent Symptomatic Control ไตไม่ดี  Steroid low dose (<10 mg/day) ไตดี  NSAIDs, Colchicine Symptomatic Control Only NSAIDs, Colchicine Treat cause!!! Urate lowering Agent Start when Attack > = 2/ yr , Tophus , Bony erosion or Urolithiasis , Urate >= 13 mg/dl in male or >= 10 mg/dl in female Check for Metabolic disease! Ca, Mg, SI, TIBC, Ferritin, UA, PTH ( H emochromatosis, Hyper parathyroid , Hypomag nesemia ) Overhanging edge , Bony erosions as arrow Chondrocalcinosis yellow arrow Subchondral bone sclerosis blue arrow

RA 2 ข้าง, 3 ข้อ, มอร์นิ่ง > 1hr , ติ่งย้อย, RF , มักโดนข้อมือ (spare DIP!), ภาพรังสีเข้าได้, อาการ > 3-6 wk OA คนแก่ๆทั่วไป ถ้าอายุน้อยอย่าลืมหา cause เป็นได้ทุกข้อ มากที่สุดตามข้อที่ลงน้ำหนัก เช่น เข่า มักมี Extraarticular !!! ตาม ผิวหนัง ปอด ไต มีแค่ข้อเท่านั้น ตรวจ RF, Anti-CCP, film hand& wrist& lateral C-spine !, Boutonniere (PIP flex, DIP ext ), Swan neck ( ตรงข้าม ) ulnar deviate, C1-C2 subluxation ตรวจแค่ X-ray เท่านั้น Bouchard’s Node (PIP) Heberden’s Node (DIP) DMARDs  Methotrexate (folate def , hepatotoxic) , Biologic Anti-TNF เช่น Infliximab Rapid acting  NSAIDs, Steroid (low-dose) Start DMARDs + Rapid acting พร้อมกัน Symptomatic Control Only NSAIDs, Opioid ห้ามให้ Steroid เด็ดขาดยิ่งเร่งข้อพัง !!! ไม่ไหวก็ ผ่าตัดเปลี่ยนข้อ Bony erosions และ periarticular osteopenia ( เส้นสีขาวๆ ) as arrow Narrow joint space มัก medial side, Osteophyte, Subchondral bone sclerosis and cyst
