INTRODUCTION MRI: An MRI provides an accurate picture of the structures of the brain using magnetic technology. EPILEPSY: An epilepsy protocol mri is different from a standard brain mri because the pictures are focused to look in the structures of brain that are most likely to cause seizures.

INDICATIONS Onset of partial seizures, at any age. Onset of generalized or unclassified seizures in the 1 st year of life. Difficulty obtaining seizure control with first line antiepileptic drugs. Loss of seizure control or a change in the pattern of seizures.

SEQUENCES USED IN EPILEPSY PROTOCOL Localizer. T2 axial. T2 flair axial. T2 coronal flair oblique. DWI axial. MPRAGE .

LOCALIZER A three plane localizer must be taken in the beginning to localize and plan the sequences. A localizer must be less than 25seconds.

T2 axial Plan the axial slices on sagittal plane . Angle the position block parallel to the genu and splenium of the corpus callosum. Slices must be sufficient to cover the whole brain from the vertex to the line of the foramen magnum. Check the position block in other two planes. An appropriate angle must be given in coronal plane on a tilted head(perpendicular to line of 3 rd ventricle and brain stem).

PARAMETERS and images TR- (3000-4000). TE-(100-120) Slice thickness: 5mm Flip angle-(130-150) Matrix size-(320x320) FOV: (210-230 ) GAP:10% NEX:2

T2 coronal flair oblique(epilepsy protocol) Plan the coronal high resolution slices on the sagittal plane. Angle the position plane perpendicular to the long axis of hippocampus. Check the position block in other two planes . An appropriate angle must be given in the axial plane (perpendicular to the mid line of the brain). Slices must be sufficient to cover the whole temporal lobe.

PARAMETERS and images TR- (3000-4000). TE-(100-120) Slice thickness: 2mm Flip angle-(130-150) Matrix size-(320x320) FOV: (180-200 ) GAP:10% NEX:4 Phase(A>P)

DWI Diffusion weighted imaging (DWI) detects changes in the distribution of water molecules by measuring the apparent diffusion coefficient (ADC) and is mainly used in the diagnosis of ischemic stroke

MPRAGE The most used post-GBCA sequence is MPRAGE, an inversion recovery fast gradient recalled-echo sequence. This technique emphasizes the anatomic brain tissue contrast between gray and white matter due to the inversion recovery preparation pulse . It can demonstrate the high-resolution 3d magnetization prepared rapid acquisition with gradient echo. MPRAGE pulse sequence has been well described and is now commonly used in clinical imaging of brain.
