NATIONAL MENTAL HEALTH PROGRAM PRESENTED BY A. Hemalatha MSC[N] 2nd YEAR GCON, CUDDALORE
INTRODUCTION Mental health is an integral & essential component of health . The govt of India launch the national mental health programme in 1982.keeping in view the heavy burden of mental illness in the community & inadequate of mental heath care infrastructure in the country to deal with it.
HISTORY OF MENTAL HEALTH PROGRAMME In 1980 the government of India felt necessity of evolving the plan of action aimed at the me n tal health component of the national health programme. In February 1981 a drafting committee met in Lucknow and prepared the first draft of NMHP. This was presented at a workshop at New Delhi on 22-21 July 1981.
Continued…. In August 1982 the highest policy making body in field of health in the country, the Central council of health & family welfare adopted & recommended for implementation of NMHP.
DEFINITION OF MENTAL HEALTH Mental health is a state of well being in which an individual realize his/her own abilities can cope with the normal stresses of life, can work productively and is able to make a contribution to his/her community WHO
DEFINITION OF MENTAL ILLNESS A mental illness is a medical condition that disrupts persons’ s thinking feeling , mood, ability , to release to others and daily functioning. SREEVANI
AIMS OF NMHP Prevention & treatment of mental neurological disorders and their associated disability. Use of mental health technology to improve general health services. Application of mental health principles in total national development to improve quality of life.
OBJECTIVES OF NMHP To ensure availability & accessibility of minimum mental health care for all in the foreseeable future, particularly to the most vulnerable & under privileged sections of the populations. To encouraged application of mental health knowledge in general health care social development. To promote community participation in the mental health services development & to stimulate efforts towards self help in the community.
STATEGIES OF NMHP Integration of mental health with primary healthcare through the NMHP Provision of tertiary care institutions for treatment of mental disorders. Eradicating stigmatization of mentally ill patients & protecting their rights through regulatory institutions like the central mental health authority & state mental health authority.
APPROACHES OF NMHP Integration of mental healthcare services with the existing general health services. Utilisation of the existing infrastructure of health services & also deliver the minimum mental health care services. Provision of appropriate task oriented training to the existing health staff. Linkage of mental health services with the existing community development programme.
COMPONENTS OF NMHP
TREATMENT
VILLAGE & SUBCENTER LEVEL 1.Training for: Multipurpose workers Health supervisors Medical officers 2.Management of psychiatric emergencies. 3.Administration & treatment for chronic psychiatric disorders. 4.Diagnosis & management of epilepsy , especially in children.
Continued … 5.Liasion with local school teachers & parents regarding mental retardation & behavioural problems in children. 6.Counseling problems related to alcohol & drug abuse. PHC LEVEL M. O. of PHC trained for: 1.supervision of MPHW performance. 2.Elementary diagnosis.
Continued… 3.Treatment of functional psychosis 4.Treatment of uncomplicated cases of psychiatric disorders . 5.Management of uncomplicated psychosocial problems. 6.Epidemiological surveillance of mental morbidity.
District level hospital The district hospital should have 30-50 psychiatric beds. One psychiatrist attached in each district hospital. His part of work is clinical care & main part of work is training & supervision of non – specialist health workers.
Medical college level Help in care of difficult cases. Teaching Specialized facilities like: 1.occupational therapy 2. psychotherapy 3.counselling 4.behavioural therapy.
REHABILITATION Development of rehabilitation centres at district level & higher referral centres. Treatment of epileptics& psychotics. Prevention Prevention & control of alcohol related problems. Juvenile delinquency Suicidal attempts .
RESTRATEGIES OF NMHP The NMHP was re strategied in 2003.[10 th five year plan]. Extension of DMHP in 100 districts . Upgradation of psychiatric wing of medical colleges. Modernization of state mental hospital . IEC activities. Monitoring & evaluation.

SERVICES PROVIDED UNDER DMPH Clinical services &out reach services Training all the ground level workers in identifying & referring patients with mental illness. Training all the M.O. s to identified & start first line treatment for mentally ill. IEC activities. Counselling services in schools & colleges. Work place stress management Suicide prevention services
ROLE OF CHN IN NMHP PRIMARY PREVENTION Health education related: parenting skill & child development to new parents Technique of stress management Physical & psychosocial effects of school & college students. Way to cope with changes of different maturational stages & transitional periods. Concept of mental health. Provide support to unemployed & homeless.
Contd … SECONDARY PREVENTION On going assessment of risk group Care for individuals with symptoms Referral services TERTIARY PREVENTION Helps the client to learn socially appropriate behaviour. Teach about ADL. Refer to after care centres[support group, day care centres, half way homes etc.]
CONCLUSION It is possible to develop a national mental health program but it has been a gradual process. The developments in the area of mental health has brought mental healthcare from the closed confines of mental hospitals to the larger community .

(2021) National Mental Health Programme needs to be critically analysed in terms of its success, failures . experiences and lessons learnt, so as to further strengthen mental health services in our country to cope up with the ever-rising burden of mental health illnesses in our country.
