Patientassignment
In the patient method of assignment a nurse
is a expected to give complete nursingcare
to one or more patient including general
nursing measures, treatments, medications,
taking temperature, pulse,respiration

Nursing assessment is the
gathering of information about
apatient's physiological,
psychological, sociological, and
spiritual status by a licensed
RegisteredNurse.Nursing
assessmentis the first step in
thenursingprocess. A section of
thenursing assessment may be
delegated to certifiednursesaides.

SPECIFICOBJECTIVES:
At the end of this topic the student will be
able to:
Clarify purposes of nursingassignment.
Identify principles of nursingassignment.
Determine factors to be considered in
distributing duties.
Differentiate between different methods of
nursingassignment.

Purposes:
-To provide patient with best possiblenursing



care.
To provide a well rounded education
experience for studentnurses.
-Suggest appropriate procedures for nursing
care, with maximum efficiency with minimum
effort.
-Gain the cooperation of workers inthe

acceptance of work to bedone.
-Serve as workplan.

Formulate assignment of activities based onthe
followingprinciples:
-Use simple clear words in writingassignment.
-Fix responsibility for all duties clearly with no
overlapping.
-Provide for emergency situations and provide for
special nursingactivities.
-Give each person time and opportunity to plan her
work.

-Assignment must include information from the
nursing orders as well as from the doctor'sorders.
-Arrange the various partsof theassignmentin
logicalorder.
-Ensure continuity of patientcare.
-Assignment should be made in accordance with the
administrativepolicies.
-It should be made by the headnurse.
-Planning for skillsdevelopment

Methodsofassigning
personnel:
There are five methods of assignment in
generaluse:

.Functionalmethod-2
.Team nursingmethod-3
.Primary caremethod-4
.Modularmethod-5

1-Thecasemethod:
-Is the oldest patient care deliverysystem.
-In this system one must assume total
responsibility for the complete care of the
patient while she is onduty.
-The patient has different nurse in eachshift.
-Is used frequently in intensive careunit.
-As a method for teaching studentnurses.
-Is widely used in home healthagencies.

Advantages:
-Quality of patient care ishigh.
-Patient satisfaction Ishigh.
Disadvantage:
-Limited continuity ofcare.
-Highcost.

2-Functionalnursingmethod:
Workassignmentaredividedintotasksandassigned
tonursingandancillarypersonnel.ItIsataskfocus.

Advantages:
-Need less equipment and
supplies.
-It is efficient when there is
shortage ofstaff.
-Costeffective.

Disadvantages:
-Fragmentedcare.
-Noonenurseisfamiliarwith
patient totalneeds.
-Itisdifficulttofix
responsibility forerrors.
-Depersonalized patientcare.
-Less satisfaction for nurses and
patients.

3-Teamnursingmethod:
-Team nursing uses a group of health care
workers with varied skills, and training. They
are directed by a leader to provide total
services for a defined patientpopulation.
-Team nursing usually refers to large group of
patients.

-Theteamleaderplans,coordinatesand
supervises,evaluatesandparticipatesinthe
delivery of care topatients.
-Themainfeaturesofteamnursingarethe
nursing care plan and teamconference.

Advantages:
-High quality ofcare.
-Nurses satisfaction ishigh.
-Patients and physician satisfaction
ishigh.
-Development of leadershipskills.

Disadvantages:
-Very highcost.
-More time needed by the team to meet
andshare ideas and coordinate
efforts.
-Need more equipment andsupplies.

4-Modularmethod:
It is a combination of primary method
and teammethod.
-Varied from primary nursing that, it can be
used when nursing staff includes practical
nurses and professionalnurses.

-It is similar to primary assignment in that
each pair of nursing personnel is
responsible for care of patient from
admission todischarge.
-It is similar to team nursing that
professional and non professional workers
worktogether.

5-Primarynursingmethod:
-Primary nursing features a registered nurse who
gives total patient care from four to six
patients, while she is on duty; and remains
responsible for the careofthesepatients
twentyfourhoursadaythroughout the
patient'shospitalization.

-Theassociatenursecareforthepatientsbyusingthe
careplandevelopedbytheprimarynurse.Shehas
tocontactprimarynurseregardingchangeinthe
careplan.
Advantages;
-Increases satisfaction of both nurses andpatients.
-High qualitycare.

Disadvantages:
-Timeconsuming.
-Highcost.
