Nursing Diagnosis #1: Ineffective Airway Clearance (3-Part) Subjective Data: “I can’t breathe properly.” “I feel tightness in my chest.” Objective Data: Wheezing on auscultation Use of accessory muscles Cough with thick sputum RR = 28 breaths/min O₂ sat = 89% on room air
Diagnosis: Ineffective airway clearance related to bronchospasm and increased mucus as evidenced by wheezing, dyspnea, and ineffective cough Goal: Patient will maintain a clear airway with normal breath sounds and effective coughing within 24–48 hours.
Intervention 1. Monitor respiratory rate, breath sounds, and O₂ saturation every 2–4 hours 2. Encourage fluid intake if not contraindicated 3. Administer prescribed bronchodilators and corticosteroids 4. Teach patient to perform deep breathing and coughing exercises 5. Position in high Fowler’s position Evaluation: Patient is coughing effectively, O₂ sat improved to 95%, breath sounds clearer, and respiratory rate decreased to 20 bpm.
