Medical superintendent DUTIES OF MEDICAL SUPERINTENDENT To take daily hospital rounds supervision and attend to patient and staff grievances, inspect drugs for stock positions of medicines and other items as per requirement, arrange for replacement and informed to M.S. Check availability of M.O.'s. on daily basis, supervision of work of all departments of hospital will be assigned to M.O.'s. Routine administrative work, grant leave to all categories to staff, arrangement of substitute as per need. Supervision of O.P.D. checking of records, admissions, discharge, 0. T list, waiting list, etc. Supervise working and attendance of Office staff, Honoraries, R.M.O.'s and M.O.'s. Lab. NURSING CARE:- 1. Assist the total needs of patients in the unit and prepare planned nursing care. 2. Demonstrate and supervise the nursing care of patients in the unit. 3. Attend regular round in the unit with the medical and nursing personnel. 4. Reviewing reports from Senior Nursing Officer regarding the nursing care of patients in each shift. 5. Make indents for drugs, surgical supplies, and all other store items and issue. Keep custody of dangerous drugs/ narcotic drugs and record of their administration. Daily check of emergency drugs, dangerous/ narcotic drugs, crash Maintenance of stock registers, inventories. Assist to investigate complaints, if any.

Organization of nursing services at Community level Community health service is concerned not only with people who are sick but also with the healthy young and old, male and female. Community health and community health nursing (CHN) draws knowledge and practices from other disciplines such as medicine, surgery, paediatrics, obstetrics, gynaecology, dentistry, health education and vital statistics. The community centre may refer to a patient directly to the state-level hospital or the nearest medical college hospital. The community health centre has the following staff, which is collectively called a community health team

Medical officer A medical officer of all functions in organizer, supervisor, coordinator and evaluator all in primary health centre (PHC) And visits in the afternoons be outpatient (OP) patients in the morning and does field He visits subcentres on fixed days and hours. He gives guidance to the team.

Female health worker Registers and cares for prenatal and postnatal mothers at home Registers and follows up all eligible couples Provides nutrition advice and immunization to mother and children Refers mothers and children at the time of need to hospitals and follows up with them after discharge Carries out family planning services including the distribution of contraceptives Provides treatment for minor ailments Notifies communicable diseases Maintains records and registers all services provided and vital events such as births and deaths  

Male health worker Conducts surveys of the sub centre areas and maintains records of all families, Maintains information of all vital events Participates in the Malaria Control Programme Participates in family planning services by keeping a list of all eligible couples; provides information on those accepting family planning Participates in nutritional programmes Promotes health education activities Identifies and reports about communicable diseases Coordinates activities with female health worker and block staff Maintains records Provides generalized or multipurpose service at home, in school and in the work place Functions in the field of administration and supervision, education, training personnel, and health services and research

Community health nurse A community health nurse is a person serving at the community level and providing basic promotive, preventive, curative and rehabilitative services directly to the community. Qualities of community health nurse A qualified community health nurse is one who has undergone basic nursing training course as degree or diploma along with desirable field experience during his/her training. A community health nurse must understand dynamics of human relationships and behaviour. Community health nurse should exhibit sincere behaviour and empathy with their clients in community. Community health nurse should possess leadership skills with a generalized acceptable approach. The minimum essential skills of a community health nurse are observation,, communication interviewing, and bedside supportive and technical skills at home or at the health centre, The community health nurse must have abilities to make interpretations and judgements to take decisions related to their services to people.

At the community level nursing services deliver by main three centers

The community health centers' are established and maintained by state government under MNP/BMS [Minimum Need Programme /Basic Minimum Service] programme. It has 30 indoor beds with x-ray labour room, operation theatre, and laboratory facilities. It is managed by four medical specialists i.e. surgeon, physician, gynecologist and pediatrician. On 31 march 2003, 3076 CHC were established each covering a population of 80000 to 1.20 lakh.        

Definition Community health centers are the nonprofit community governed health organizations that provide primary health care, health promotion and community development services, using them inter disciplinary terms of health providers Community health centres are care facilities with the goal of increasing access to crucial primary and preventative care services. CHCs primarily serve patients from lower income areas who may be uninsured or underinsured, lack access to transportation, or face language barriers.

Community Health Centers has to provide the following services like Care of routine and emergency cases in surgery. Care of routine and emergency cases in medicine. 24 hour delivery services. Essentials of emergency obstetric care. Full range of family planning services including laparoscopic services. Safe abortion services. New born care. Routine and emergency care of sick children. Other management of medical and accidental conditions. All the national health programmes should be delivered through CHCS.

Functions of community health nurse Community health nurses provide comprehensive health care to individuals, families and groups by teaching, counselling and providing guidance. A community health nurse develops goals to meet the requirements. She develops an action programme, evaluates progress and plans again as needed. She gives assistance to the family in improving environmental conditions that affect health and helps to plan a safe environment in the home, school and industry. She provides supportive services to doctors, such as early symptom detection and technical help. She demonstrates and teaches the skills for taking care of the sick at home. She supervises the work of midwives, dais and other nursing personnel.

PHCS (Primary Health cenres) Introduction The PHC is the first contact point between the village community and the medical officer. These are established and maintained by the state government under minimum needs/basic minimum services programme. It acts as a referral unit for six sub centre and has 4-6 beds. A PHC covers population of 30000 in plain area and 20000 in hilly remote and tribal area. The activities of PHC's involve curative, preventive, promotive and family welfare services. There were 23,391 PHCs functioning as on March 2009 in the country .

Standards of Primary Health Centre The IPHS(Indian Public Health Standards) for Primary Health Centers has been prepared keeping in view the resources available with respect to functional requirement for PHCs with minimum standards example:- Building Man power Instrument Equipments Drugs Other facilities The standards prescribed are, a PHC covering 20000-30000 minimum population with six beds on well the block level PHC are ultimately going to be upgraded as CHC with 30 beds of providing specialized services.

Sub-Centre The Sub-Centre is the most peripheral and first contact point between the primary health care system and the community. Sub-Centers are assigned tasks relating to interpersonal communication in order to bring about behavioral change and provide services in relation to maternal and child health, family welfare, nutrition, immunization, diarrhea control and control of communicable diseases programmes. The Sub-Centers are provided with basic drugs for minor ailments needed for taking care of essential health needs of men, women and children.

There are 1, 45,272 Sub Centers functioning in the country as on March 2007. Currently a Sub-centre is staffed by one Female Health Worker commonly known as Auxiliary Nurse Midwife (ANM) and one Male Health Worker commonly known as Multi Purpose Worker (Male). One Health Assistant (Female) commonly known as Lady Health Visitor (LHV) and one Health Assistant (Male) located at the PHC level are entrusted with the task of supervision of all the Sub-centers, generally six subentries under a PHC. The Ministry of Health & FW, GOI provides assistance to all the Sub-centers in the country since April 2002 in the form of salary of ANMs and LHVs, rent (if located in a rented building) and contingency, in addition to drugs and equipment kits. The salary of Male Health Worker is borne by the State Governments.

The objectives of Indian Public Health Standards for PHCs are To provide comprehensive primary health care to the community through the PHC To achieve and maintain an acceptable standards of quality of care To make the services more responsible and sensitive to the needs of the community Minimum requirements are The assured services cover all the essentials of preventive, promotive, curative and rehabilitative primary health care. This implies a wider range of services that includes Medical care Maternal and child health care Full range family planning services including counseling and appropriate referral for couples having infertility MTP services Health education for prevention and management of malnutrition, anemia and vitamin A deficiency and coordinates with ICDS School health services
