Definition
▪ It is an infection & Inflammation of middle ear that
most often occur in infant and young children but can
occur at any age
▪Most common in children under 15 years of age
▪ It may be acute, chronic & serious Otitis media

Acute suppurtive otitis media (ASOM)
Definition
▪ It is an acute infection of middle ear which causes inflammatio
& purulentfluid accumulation ; usually lasting less than 6 weeks
Etiology
▪ Bacteria (eg) streptococcus pneumoniae
▪ Upper respiratory tract infection
▪ infection of nasopharynx(tonsilitis,common cold)

Pathophysiology
1.Due to etiology factor (URTI, Bacteria)
2. serous exaudates & edema in middle ear
3. pus formation
4. Tympanic membrane rupture
5. Otitis media

Clinical features
▪Otorrhea
▪ otalgia
▪Fever
▪Rhinitis
▪ Tympanic membrane erythema may be perforated
▪ Hearing loss and irritability

Diagnostic Evaluation
▪ History collection
▪ Physical examination
▪ Otoscopic examination
▪ Audiometry test
▪ Tympanometry

Management
Medical Management
• Antibiotics - Amoxicillin 75-90mg/kg/day
• Analgesic - Ibuprofen 1.25ml drops, 100mg tablet
• Antihistamine - Levocetirizine Dihydrochoride syrup
60ml

Surgical management
▪ Myringotomy or Tympanotomy (Incision in the
tympanic membrane to relieve pressure caused by
excessive buildup of fluid, orto drain pus from the
middle ear)

Chronic suppurtive Otitis Media (CSOM)
Definition
▪ It results from repeated infection in middle ear
causing permanant peforation of tympanic membrane .
▪ The infection involves middle ear with mastoid cells

Etiology
• Inappropriate treatment of acute otitis media
• URTI, Allegric rhinitis
• Eustachian tube deformity
• Sinusitis

Suppurtive +Perforation
a) Atico-antralchronic otitis media
Inflammation involves bones (e.g., mastoid, tympanic ring,
ossicles)
b). Tubo-tympanic otitis media
Acute otitis media permanent
Perforation Muco-purulent discharge

Clinical features
• Hearing loss
• otorrhea
• Low pitched tinnitus

Management
Medical Management
• careful suctioning of the ear under microscopic guidance
• Instillation of antibiotic drops
Surgical management
• Tympanoplasty(reconstruction of the eardrum )
• Mastoidectomy (removes diseased mastoid cells)
