PALLIATIVE CARE & HOSPICE CARE Presented By: Mr. NANDISH.S Associate Professor Mandya Institute of Nursing Sciences
DEFINITION : It is a specialized form of medical care that focuses on providing relief from the symptoms & stresses of a serious illness. Palliative care is appropriate at any stage of a serious illness. AIM : The primary aim is to manage pain & other distressing symptoms. It addresses emotional, social and spiritual aspects of patient. To improve the quality of life for both patient and their family.
INDICATIONS : Serious illness complex or progressive disease. (advanced d ementia, end stage renal disease). Multidimensional needs (it focuses on physical, emotional, social and spiritual needs) End of life care Symptom management Advanced care planning.
ASPECTS : 1. Holistic Approach : Physical care : managing pain & other symptoms through medication, therapies and other interventions. Psychological care : counselling, coping strategies. Social care : providing assistance as needed (family dynamics, social groups) Spiritual care : offering support based on individual beliefs & values. Cultural care : it involves recognizing & respecting the diversity of patients, cultural backgrounds, beliefs and traditions.
2. Pain & Symptom Management : Assessing & treating the pain to enhance comfort and quality of life. Using multidisciplinary approach, involving physician, Nurse, pharmacist and other health care professionals. 3. Communication : Facilitating open & honest communication between health care providers, patients & their family members about illness. Ensure that individuals & their families are informed and involved in decision making process.
4. Advance care planning : Discussing and documenting preferences for medical care, including the use of life sustaining treatments in advance. 5. Care Coordination : Coordinating care among health care providers, specialists and support services. Collaborating with other health care professionals (oncologist, cardiologist and physicians)
6. End – of – Life care : Preparing and supporting individuals & their families for dying process. Providing comfort care, emotional support & assistance with practical matters during final stages of life. 7. Quality of Life Enhancement : Focusing on the activities and interventions that improve the overall quality of life of patient such as music therapy, art therapy or complementary therapies.

HOSPICE CARE : It refers to a specialized type of health care focused on providing support & comfort to individuals who are nearing the end of their lives. AIMS : The primary goal of hospice care is to enhance the quality of life for patients with terminal illness. To offer emotional & practical support for both patient and family.
CHARACTERISTICS : Holistic Approach : It considers not only physical symptoms of illness, but also emotional, social & spiritual aspects of individual & their family. 2. Team Based C are: It is typically delivered by an inter disciplinary team that includes physician, Nurses, social workers & volunteers. This team collaborates to meet the diverse needs of patient & their family.
3. Patient and Family – C entered care : The plan of care is developed in consultation with patient and their loved ones, taking into account their goals, values & cultural preferences. 4. Home based care : It can be provided in various setting. This allows patients to receive care in comfort of their own homes, surrounded by familiar surroundings & loved ones. 5. Respite Care : It offers respite services, providing short term relief to family members. This allows care givers to take a break while patients to receive care.
6. Bereavement Support : This include counselling, support groups and resources to help family members cope with grief and loss. 7.Flexible care : Needs of patient can change over time. Hence care planning must be flexible and adjustable to accommodate evolving physical, emotional and spiritual needs. 8. Dignified Death: It aims to facilitate a dignified and comfortable death. This include supporting the patient’s wishes regarding end – of – life care and ensuring that their values & beliefs are respected.
