PAP SMEAR REPORTING

SPECIMEN TYPE

Conventional pap smear or liquid based preparation

ADEQUACY?

Minimum squamous cellularity criteria : 5000 well visualized / well preserved squamous cells in liquid based preparation . 8000 – 12000 cells in conventional preparation . Endocervical / Transformation Zone component : 10 well preserved endocervical / squamous metaplastic cells singly / in clusters .

SPECIMEN ADEQUACY Satisfactory for evaluation Unsatisfactory for evaluation Specimen rejected/not processed Specimen processed and examined, but unsatisfactory for evaluation of epithelial abnormality

More than 75% of cells obscured by inflammation, bacteria or interfering substances (lubricants and blood)

Woman's postchemotherapy, radiotherapy, postmenopausal, atrophic changes or post hysterectomy may have < 5,000 cells

Exception: adequate if any abnormal cells are present

General categorization (optional) Negative for intraepithelial lesion or malignancy Epithelial cell abnormality: see Interpretation / result (specify squamous or glandular)

Non neoplastic cellular findings: Reactive cellular changes with association specified: Inflammation (with or without repair) Lymphocytic (follicular) cervicitis Radiation Intrauterine contraceptive device changes

ORGANISMS Trichomonas vaginalis Fungal organisms morphologically consistent with Candida species Shift in flora suggestive of bacterial vaginosis Bacteria morphologically consistent with Actinomyces species

EPITHELIAL CELL ABNORMALITY Epithelial cell abnormalities Squamous cell Atypical squamous cells Of undetermined significance (ASC-US) Cannot exclude HSIL (ASC-H)

Low grade squamous intraepithelial lesion (LSIL) High grade squamous intraepithelial lesion (HSIL) With features suspicious for invasion (if invasion suspected) Squamous cell carcinoma

Glandular cell Atypical Endocervical cells (NOS or specify in comment) Endometrial cells (NOS or specify in comment) Glandular cells (NOS or specify in comment) Endocervical cells, favor neoplastic Glandular cells, favor neoplastic
