¶ INTRODUCTION:- A medication history is a detailed, accurate and complete account of all prescribed and non-prescribed medications that a patient had taken or is currently taking prior to a newly initiated institutionalized or ambulatory care. The documentation of medication history has historically been undertaken mainly by physicians and sometimes nurses. The participation of pharmacists in the documentation of medication history has been shown to result in significant improvement in its accuracy and comprehensiveness.
¶ NEEDS FOR THE PATIENT MEDICATION HISTORY INTERVIEW:- • Compare medication profiles with the medication administration record and investigate the discrepancies. • Verify medication history taken by other staffs and provide additional information where appropriate. • Screen for drug interactions. • Assess patient medication compliance. • Assess the evidence of drug abuse. • Examine the needs for medication aids.
¶ IMPORTANCE OF ACCURATE DRUG HISTORY:- • Medication histories are important in preventing prescription errors and consequent risks to patients. • Apart from preventing prescription errors, accurate medication histories are also useful in detecting drug-related pathology or changes in clinical signs that may be the result of drug therapy. • A full medication history identifies patients’ needs and explores the Patient's perspective of illness and its treatment (needs and concerns).
¶ DATA TO BE OBTAINED IN PATIENT MEDICATION HISTORY INTERVIEW:- Data obtained during the medication history interview include : Demographic information, Dietary information, Social habits, Current and past prescription medication, Current and past Non- prescription medications, Current and past complementary and alternative medicine, Drug Allergies, Adverse drug reactions, Immunizations, Adherence to Prescribed or recommended medication regimens.
¶ PATIENT MEDICATION HISTORY INTERVIEW FORMS:- The meditation history is the part of the patient interview that provides the pharmacists the opportunity to utilize his or her expertise by precisely collecting each component of the medication history. The questions that you ask the patient,as well as the technique used, will enable you to learn exactly how, when and why a patient takes each medication. as well as about any adverse reactions, allergies, or issues with medication cost akespatient may have experienced.
