Peptic ulcer is a disease caused due to breaking of inner lining of stomach.
There are two types of peptic ulcer :


●The ulcer caused in duodenum is called as duodenum ulcer.
●It is most common type of ulcer
●The ulcer caused in stomach is called as gastric ulcer.

Gastric acid secrtion inhibitors:
1.Proton pump inhibitors : Omeprazole, Lansoprasole, Pantoprazole,
Rabeprazole, Esmoprazole, Dexrabeprazole
2. Anticholinergic : Pirenzepine, Telenzepine, Propantheline,
Oxyphenonium
3.H2 receptor antagonist : Cimetidine, Ranitidine, Famotidine, Roxatidine,
Loxatidine
4. Prostaglandin analogue : Misoprostol, Enprostil, Rioprostil
1.Ulcer causes due to infection with Helicobacter pylori bacteria.
2. Use of drugs like NSAIDS also causes ulcer.
3. Excessive secretion of acid.
4. It can also caused due to viral infection.


Etiology

Intake of drugs like Reserpine, Corticosteroids [NSAIDS], alcohol, tea, coffee

Decrease mucus secretion and excess secretion of HCL

Breakdown of inner lining [mucus membrane] of stomach

Due to which acid enters into gastic mucosa

Pathophysiology

Mechanism of action :
●Proton pump inhibitors inhibit the proton pump
[H+K+ ATPase] which is stimulated by gastric
parietal cells & secretion of HCL is minimised.
●H2 receptor is responsible for increasing acid
secretion which activates H+K+ATPase by
generating cAMP by action of histamine, H2 blocker
Is given which decrease the acid secretion.
●Gastric mucosa produces PGE2 [ prpstaglandinE2]
Which is responsible for inhibiting acid secretion by
inhibiting cAMP generation.
●Bacteria H pylori is major cause for peptic ulcer by
causing
Hypergastrinemia, so for this anti H pylori drugs are
given.
