Purpose of Permit To Work System (PTW) A permit to work system is to ensure that All works are approved and carried out under the possible condition. All relevant personnel are aware of all work in progress in their areas on responsibilities Personnel are aware of their responsibilities / authorities in carrying out any work activities in the work area through the observance of the rules set out in these procedures so as to Safe guard the worker and personnel safety and well being Prevent fires, accidents or any incidents Protect the property, equipment and operations 3

Permit To Work Safe work permits (PTW) are issued by local operating personnel to control the work in operating areas and ensure that maintenance and/or construction personnel are aware of safe work practices. 4

PTW Implementation Process – Issuer & Applicant 9 Review permit and understand the limitations. Sign the permits after satisfied with safety precautions proposed for the work. Permit Issuer Review permit conditions, precautions and limitations /Stop work if any changes in condition. Ensure employees understand terms used in the permit. E.g. hot work, inert, safety standby purge…etc Applicant

Role of Supervisors Thoroughly “ Familiar With Safety Requirements ” and “ Procedure For Obtaining Work Permits ”. Ensure employees/Workers are familiar with the “Requirements For Work Permits”. Coordinate all work with the Client/PMC representative ensuring that: A properly filled out work permit is issued, The hazards and limitations at the site are assessed, A joint inspection of the area and positive definition of the work is accomplished, and All precautions are taken and safe practices are followed. Before work begins the supervisor shall collaboratively develop a Safe Plan of Action (SPA) with his/her employees and assure that they understand any potential hazards and the methods to eliminate or control the hazards. 10

Roles and Responsibilities – Employees Obtain the “Proper Authorization” and “Appropriate Work Permit”. Must perform work within the scope/limitations outlined on the written work permit. Must heed all permit warnings, precautions, and wear the required personal protective equipment. Stop work immediately and notify supervision, if any changes in job conditions take place . 11

Contractor – Prepare Method Statement (MS) &SPA and get approval Contractor – Apply for work permit by enclosing MS & SPA Issuer – Visit site, list out precautions, restrictions including contractor interface conditions 2 Issuer & Applicant Sign work permit form 6 Applicant Comply with all requirements in the checklist and start the work

Sequence of Work Permit – 10 Step Process 13 7 10 9 Contractor – Can only leave the site once permit has been signed off and cleared Contractor – Display “Work Permit” and “SPA” in work location Validity of Permit Time frame (Renew day/shift) No change in conditions and environment Issuer – Once job is completed the permit must be signed off 8

Permit Renewal and Validity Period Permit shall be renewed after each shift or beginning of each day (Subjected to condition mentioned in the permit) Blanket permit shall be renewed after expiry of validity period (if any work is pending) 14

Handing Over/Returning of Permit Work permit must be returned to the issuer after completion of work or end of each day/shift or as mentioned in the permit. 15

Effectiveness of PTW System The supervisor shall make periodic inspections of employees permitted work activity. Safety Observation Reports (SORs) shall be used by the supervisor when safety deficiencies are observed. 16

Types of Work Permits & Formats

Permit To Work (PTW) System 19 Permit To Work (PTW) System Confined Spaces – Entry permits shall be issued when company employees are required to enter a confined space The confined space — even if vessels, pipe, etc., are clean and have never contained product — shall be tested for combustible gases, inadequate oxygen, or toxic atmospheres . After turnover of the equipment to the client/Jacobs, the client/Jacobs will issue their confined space entry permit when employees are required to re-enter the confined space for any purpose. Line Breaking – Permit is required before opening process lines or equipment containing materials that may pose a hazard to workers. Hot Work System – Permit shall be obtained before working in operating facilities when work involves welding, burning, grinding, or electrical, this may include of photo-taking in highly flammability substance storage /usage plant

It is a control measure to ensure that all safety checks are in place before work begins, particularly for high-risk tasks like working at height, hot work, confined space entry, or SIMOPS (Simultaneous Operations).

Lockout Tagout(LOTO) Oct`2024

Purpose To ensure the machine or equipment is stopped, isolated from potentially hazardous energy source and locked out before employees perform any commissioning or installation which, if unexpectedly energized or started, could cause injury. 23

Definition Energy isolation device – A device that physically prevents the transmission or release of energy Lockout – The placement of lockout device (usually a lock) on an energy isolating device is to ensure that the equipment being controlled Cannot be operated. 24

Definition Tag Out – The placement of a tagout device (usually a tag) on an energy isolating device is to indicate that the equipment being controlled Cannot be operated until the tagout device is removed. 25

Definition Electrical Isolation – To ensure equipment is at zero potential in relation to ground and ensures that it remains that way for the duration of the work. 26

Testing & Verification Isolation must be verified before starting work Qualified electrician shall test all electrical power sources with an approved meter Attempt to restart to verify or otherwise verify the equipment or process cannot be inadvertently started Take necessary steps to identify, isolate and LOTO the sources until verification is positive

Communications For tasks requiring LOTO, which involve multiple shifts, the supervisor must ensure the continuity of the isolation as follows: The off going supervisor will remove his lock and tag only after the incoming supervisor has placed his lock and tag on the lock box. The incoming supervisor will ensure that the incoming craft workers verify isolation and place their locks and tags on the lock box.

General Requirements Only authorized persons are allowed to install Lockout/tagout devices. Only standardised locks & tags approved shall be used. If work is done by contractors, they should provide their own lockout/tagout devices subject to approval. 29

General Requirements Multiple locks to be used when more than one operator is working on the same job. If a piece of equipment has one source of energy or multiple lockout points exist, each authorised person working on the equipment must put a lock on each source/lockout point. 30

General Requirements DO NOT remove lockout/tagout devices installed by others. Unauthorized removal of another person’s lock or tag shall result in immediate removal from site. Name of person who installed the tags shall be clearly written on the tag & if possible the contact numbers should also be provided. 31

General Requirements A permit to work must be issued by an authorized person before work is commenced on a Lock Out/ Tag Out equipment The permit issued must be cancelled by the authorised person after removing the Lock Out/ Tag Out . A log book/ register shall be maintained of all Lock Out/ Tag Outs installed & removed by the authorized persons 32

Multiple Locking System Energy isolating devices guard against accidental machine/ equipment startup or re-energization of machine/equipment. The two types of devices are: Those capable of being locked shall use a lock. Those not capable of being locked shall use a tag. 33
