PoliceBlotterisarecordofdailyeventsoccurringwithintheterritories/jurisdiction
ofagivenpoliceunitorcommand.Itcontainsmaterialdetailconcerningtheevent
forlegalandstatisticalpurposes.Thepoliceblotterifthereforeaninformational
recordbookthatisutilizedforevidentiaryorreferralpurpose(CircularNo.5,GHQ
PNPdated10Dec1992)
EachPNPoperatingunitshallmaintainanofficialpoliceblotterwherealltypesof
operationalandundercoverdispatchesshallberecorded.
Anofficiallogbook,coveredbluewithhardboundcoverthatcontainsthedaily
registerofallcrimeincidentsreports,officialsummaryofarrestandothersignificant
events/activitiesreportedinthepolicestation.

ImportanceofthePoliceBlotter.
Blotterentryreportsserveaspermanentrecordofincident,events,problems,
andsofort.Therearemanytypesofreports,eachofwhichservesdifferent
functions.
APoliceBlotterisusedtokeeppeopleinformedofactivitieswithinthepolice
jurisdiction.itisalsousedtocompilestatisticalinformation,identifyproblemsinthe
community,oridentifypolicetrainingneeds.
Thesereportsareneededtofacilitateinvestigation,preparecourtcases,ordefend
incourt

LEGALBASISOFPOLICEBLOTTER

b.BlotterProcedure
Crimeincidents,arrestsorevents/activitiesshallberecordedintheblotterbookby
theDeskOfficer.
IfthereportisacrimeincidentasverifiedbytheDO,he/sheshallaccomplishthe
IncidentRecordForm(IRF)(Annex“NN”)usingCrimeInformationReportingand
AnalysisSystem(CIRAS)fromwhichtheentryintheblotterbookshallbeextracted
from.
IftheincidentfallsunderthejurisdictionoftheKatarungangPambarangay,itshallal
berecordedbutindicatedindispositionasreferredtoBarangay.
IfthecrimeincidentinvolvesChildreninConflictwiththeLaw(CICL),alwaysusea.k.a.
oraliaseswhenwritingentriestoblueblotter

ContentsofEntry
Theentryinthepoliceblottershouldanswerthefollowingcardinalelementsofa
policerecord,towit:Who,What,Where,When,whyandHow;andthedispositionof
thecase.
Inansweringtheabove5Wsand1Handthecasedisposition,allsuchmaterialdetails
abouttheevent,including;
a.thenameofthesuspect/s;
b.thevictim/s;theeyewitness/es;
c.thedateandtimeofoccurrence;
d.significantcircumstancesthataggravateormitigatetheeventorthecrimeshouldbe
enteredalongwiththeidentityoftheofficertowhomthecaseisassigned-officer-
on-case;and
e.thestatusofthecase.

ProceduresinMakingEntries
1.Allentriesinthepoliceblottershallbehandwritteninaclear,conciseandsimple
mannerbutansweringasfaraspracticablethe5Wsand1H.Clarityshouldnotbe
sacrificedforbrevity
2.Onlyfacts,notopinions,areenteredintheblotter
3.Noerasuresshallbemadeontheentries.Correctionsaremadebydrawingone
horizontallineoversuchwordorphrasesandtheactualentryinitialedbythepolice
officermakingthecorrection
4.Aballpenorpenwithblue,blackorblueblackinkisusedformakingtheentries
5.Misrepresentationsintheblotteroranyattempttosuppressanyinformationtherein
arepunishablecriminallyandadministratively.

ProceduresinMakingEntries
6.Theentriesmustbelegiblywritteninlonghandandconsecutivelynumbered.
7.Everypageoftheblottershallbeconsecutivelyorchronologicallyfiled-up.Noline
ofspaceshallbeleftblankbetweenanytwoentries.
8.Anydevelopmentofacasetobereflectedintheblottershouldbenewentryatthe
timeanddayitwasreported.Areferencetothepreviousentrynumberofthecase,
however,shouldbemade.
9.Ineveryshift,theDutySergeant,underthesupervisionoftheDutyOfficeror
ComplaintDeskOfficer,shallmaketheactualentriesintheblotterandattheendof
histourofduty,bothDutySergeantandDutyComplaintDeskOfficershallsignthe
blotter.

Thefollowingquestionshouldbeansweredasaguide:
a.Whowasinvolved?Writedownthenamesofallthosewhowere
involvedintheincident,victims,suspects,witnesses,andwhosoever
maybelistedaspresentduringtheincident,Thereisnothingtolose,
butalltogain,ifallthesepersonsarelisted.
b.Whendiditplace?Beprecise,asmuchaspossible,inindicatingthe
timeanddateofincident.

c.Wheredidittakeplace?Describedindetailwheretheincidentoccurred,e.g.,“In
mastersbedroomofatwo-storybrickhousein123JuanLunaStreet,VillaSanJose
Subdivision,Brgy.SanRoque,Nabua,CamarinesSur,’
d.Whathappened?Policereportssometimesindicatethecrimecommittedbasedon
theRevisedPenalCode(PRC)likeMurderorHomicide.Ininitialreports,whichwillbe
subjectedtothoroughinvestigation,thisisnotnecessary.Intheaboveexample.A
reportofa‘Shootingincident“maybesufficient.Itisthejoboftheinvestigator–on
–casetodeterminedwhatcrimewascommitted.Whatisimportantistopresentthe
detailedcircumstancesofwhathappened.

e.Whydidithappen?Thepitfallofcomingtoconclusionatthisearlyalways
committedbythepoliceresponder.TheWhy”answersaquestion.Amongothers,as
tothemotiveoftheincident.Itisnecessarytolistdownanddetailallthatwere
allegedbythevictims,suspects,andwitnesses.
f.Howdidithappened?Thenarrativeonhowtheincidenthappenedshallbeindicated
toshowthedescriptionofthecircumstancesthereafter.Theactiontakenduringthe
initialinvestigationatthesceneshallalsobeincluded.Thisshallincludethe
weather,lighting,sounds,activitiesinthevicinityoftheincident,adescriptionofthe
circumstancesprevailingbefore,during,andaftertheincident,andallotherpeculiar
detailsthatcometothesensesofthetrainedpoliceresponder.Thereisnolimitto
whattoincludeinthe“How”portionofthenarrative.

g.AffixingSignaturesandContactDetails.
AssoonastheincidentistranscribedinthePoliceBlotter,theDeskOfficerandthe
ReportingPersonshallsigninthePoliceBlotter.

metallicbearingplatenumberKGN449parkedontherightlaneofthebarangayroad
anditsdriverssidefrontdoorglasshastwoholesbelievedtobefromagunshotthus
theyimmediatelycordonofftheareaandafter-a-whileSPO4RichieB.Raiz,Chief
Investigator/MESMOarrivedattheareaandtookoverthecrimesceneasInvestigator-
on-case.

•ThatinitialinvestigationconductedbyIOCwiththeassistanceofthefirstresponders
revealedthatatabout4:00pmof08April2017,thevictimMICHAELGOMEZ,was
drivinghisaforesaidvehicletogethersaidMARIOSANTOSJR.,whoseatedonthefront
seat,traversingthebarangayroadanduponreachingatZone11,Baluarte,Tagoloan,
Mis.Or.,two(2)stillunidentifiedsuspectson-boardaMotorcyclewhowerefollowing
themapproachedthesaidvehicleonthedriver’ssideandsuddenlytheunidentified
back-riderfiredtwoshotswiththeusedofashortfirearmtothedriverssidefrontdoor
glassthushittingthedriver/victimMICHAELGOMEZonhisneck.
•Thatimmediatelyafterfiring,thetwounidentifiedsuspectsfledtheareagoingto
Poblacion,Tagoloan,Mis.Or.,whilethevictimwasbroughttoTagoloanPoliceStation
onboardamotorizedsikadbyhiscompanionMARIOSANTOSJR.,andsomeconcern
citizen.

•Thatatabout6:00o’clockintheevening,samedate.,PNPCrimelab-10personnel
headedbyPCICHISTIANKARLTERRENCECABALLESarrivedattheareaandconducted
crimesceneprocessing.
•Thatduringcrimesceneprocessing,thefollowingitemswererecoveredtowit;one(1
)firedbulletofunknowncaliberfromthedriver’sseat,one(1)unitSamsungcellular
phone,one(1)walletcontainingassortedcards,PNPandretireeID,assorted
documents/receipts,one(1)PNPbadgewithNr075819,one(1)sunglasses,two(
2)Goldring,one(1)stainlessmagazineandone(1)500pesobill.
•Thatthecrimesceneprocessingendedatabout8:00o’clockintheevening,samedate
andthepersonalbelongings/documentsandthesaidToyotaCorollaAltizwere
properlyturn-overtotheIOCforcustodybythePNPcrimelabpersonnelandwas
subsequentlybroughttothisofficeforproperdisposition.

•ThataccordingtothewifeofthevictimMICHELLEGOMEZyALCANTARAthather
husbandwasaformermemberofthePNPwhoretiredlast2009withtherankofSPO3
andpresentlyengagedinseveralbusinessesatCagayandeOroCity.

2011

InApril22,2009,theLOI02/09UnitCrimePeriodicReport(UCPER)
whichisaauniformprocedureofreportingandrecordingofcrime
incidentswasestablishedtogenerateacrediblecrimestatisticswhich
servedasthebasisinplanning,policyformulation,deploymentof
resources,andbudgeting.
Unfortunately,duringtheconductofnationwidevalidationbythe
DirectorateforInvestigationandDetectiveManagement(DIDM)in
2013to2014,severaldysfunctionswerenotedspecificallythehigh
numbersofcrimesthatwerenotaccountedfromthepoliceblotter
books.Thisrenderedthecrimestatisticsinaccurateandnotreflecting
thetruecrimeenvironment.

crimerecording,theDIDMdevelopedthePNP
CrimeIncidentRecordingSystem(CIRS)known
asthee-BlotterSystemonSeptember6,2011.
ItwasadoptedbyallunitsofthePNPmandated
thatallincidentreportsshallbeenteredtothe
system.

SalientFeatures
1.ItestablishedanStandardOperatingProcedure(SOP)onhowtoenter
dataofincidentinthePoliceBlotterbyeveryDeskOfficer,inlinewith
QualityServiceLaneProgramofthePNPtoenhancethepolicefrontline
servicestosatisfytheneedsandexpectationofthecitizenry.

complainant/victim.OnceissignedandacknowledgedbytheDeskOfficer

bemadeavailabletotheclientforhimtofillout.
4.OfficeSpaceforClients.TheQSLDutyOfficershallatalltimesguideandassistthe
clientinfillingouttheform.Intheeventthattheclientisilliterateorincapacitated
tofillouttheIRF,theQSLDutyofficershalldothesameforhim.TheCOPofthe
stationshallmakeavailableaspaceinthepolicestationwherethisprocessisdone,in
theprivacyoutsidethescrutinyofkibtzers.

9.TurnovertoPoliceInvestigator.Thereafter,itisthedutyofthepoliceofficerwho
isin-chargeofthepoliceBlottertoLodgethecomplainttotheproper
investigatorwhowillhandlethecase.Theinvestigatorshouldinterviewthe
complaintonthenatureofhiscomplaintandensurethatallentriesthereinare
completetopreventduplicationandrelievethecomplainantfromrepeatinghis
narration.
10.ManualRecordingofthecrimeIncidentReportfromtheIRFtothePoliceBlotter.
ThecrimeincidentreportintheIRF,afterthetransactionwiththereporting
Personhasconcluded,shallbetranscribedandenteredintothepolicestations
equippedwiththecrimeincidentReportingSystem,popularlyknownasthee-
Blotter,thedesignatedcrimeregistrarwilllikewiseentertherecordintothe
System.
11.EstablishIncidentRecordingSystem(BlotterProcedureFlowchart)

5.ConfidentialReports.Wheneverthesubjectofthecomplaintinvolves
confidentiality,asincasesinvolvingprotectionofwomenandchildren,theQSL
DutyOfficershallescorttheclienttothefemaleDutyInvestigatoroftheWCPD
wherethefillingoutoftheIRFshallnedone.
6.ThenarrativeoftheIncident.Thefactsoftheincidentsshouldbenarratedbythe
complainant.Itcontainsanarrativeofwhathappened,answeringthefiveW’s(Who,

7.ImportanceofSignature.Itisnecessaryandmandatoryrequirementthatattheend
ofthenarrationofeveryincident,thedutyofficerandcomplainantcertifyingthe

( CIRS ) –e-blotter -2014

InApril2014,aweb-basedCIRSwasdeveloped.
Thestand-alonesystembecameanin-house
webdevelopmentsystemandCIRSwas
renamedinto"eBlotterSystem."
CrimeIncidentReportingSystem(CIRS)-isan
electronicdatabasesystemthatfacilitatescrime
documentationandsystematicdatastorageand
retrieval.
Thesystempresentsaquick,fastandreliable
transmissionofcrimeinformationfromlower
policeunitsandofficesofthePNPtoNational
Headquarters.

DatabaseshallincludeallincidentsenteredandstoredintotheCIRS.Itis
acollectionofinformationorganizedinsuchawaythatacomputer
programcanquicklyselectdesiredpiecesofdataorganizedbyfiled,
recordandfiles.
IncidentRecordForm(IRF)–isanaccountableformtobefilledoutbya
complainant,tobeassistedbytheQSLDutyOfficer.TheIRFbecomesan
officialdocumentonceitissignedbytheinvestigatorsandthereporting
personinwhichtheinformationcontainedshallberecordedinthepolice
blotter,uploadedtotheCIRSandbecomesthefirstdocumentincludedin
thecasefolder.

QualityServiceLane(QSL)–isacustomerfriendlyqualityservicedesk
mannedbyaDeskOfficerandotherdesignatedPNCOs,underthe
supervisionoftheDutyOfficerofthePoliceStation,whichshallprovide
theinitialpointofcontactforallconcernsorproblemsofacomplainantin
thepolicestation.
Under-ReportingofCrimes–isthedeliberatenon-submissionofcrime
incidentsrecordedinthePoliceBlottersbyresponsibleofficersthat
resultedintheinaccurateanddecreasedcrimestatisticsinhisareaof
jurisdiction.

PROCEDURE
Thefollowingproceduresshallgoverntheactionstobeundertakenbykey
responsiblepersonnelintheuploadingofthecrimeincidentintotheCIRS.
a.TheDeskOfficer(DO)shallentertainandreceivethereportfromthecomplainant
andinitiallyrecordedthecomplaintintotheblotterbookindicatingtheentry
number,date,name,placeandnatureoftheincident.
b.TheDOshallinitiallydeterminewhethertheincidentisacrimeornot.Ifthe
incidentinvolvesthecommissionofacrime,thecomplainantwillbeendorsedto
theDutyInvestigator,otherwiseheshallbereferredtotheDutyOfficerwhoshall
addresshisconcern.

c.TheCrimeRegistrar,underthemandatorysupervisionoftheDutyInvestigator,shall
commencetheentryofdataintotheCIRS
d.TheDutyInvestigatorandthecomplainantshallreviewtogethertheinformationand
thesummarydetailsoftheincidentprovidedbythelatter.
e.Ifnocorrectionoradditionalinformation,theCrimeRegistrarshallimmediately
uploadthedataintotheCIRS,assistedbytheDutyInvestigator.
f.AfterstoringthefinaldataintotheCIRS,theCrimeRegistrar,withtheassistanceof
theDutyInvestigator,shallnowprintcompletedIRF.

g.ThreecopiesoftheIRFshallbeprintedbytheCrimeRegistrar,whichshallbesigned
bothbytheDutyInvestigatorandthecomplainant.
h.TheDeskOfficershallmakeadailyaccountingofallcrimeincidentsreportedinto
thepolicestationandreferredtotheDutyInvestigatorduringhistourofduty.The
DOshalllikewisefollowupandreviewthestatusoftheincidentsreferredbyhimto
theDutyInvestigator.
i.TheDutyInvestigatorshallensurethatallsignedIRFacteduponbyhimandthe
CrimeRegistrarshallbewrittendownintothePoliceBlotterbytheDO.
j.TheCrimeRegistrarshallensurethathereceivestheIRFofallcrimeincidentsfrom
theDO.

k.TherecordsintheSystemshallbeupdatedfromtimetotimebytheIOCastothe
developmentanddispositionofthecase,onceacasewasfiled,whenaWarrantof
Arrestwasissued,asuspectwasarrestedoranyotherpertinentdetailsonthe
progressofthecaselikedatesofinquest,arraignment,trialuptothefinal
promulgation.
l.Intheeventthatthereisnointernetconnectivityintheworkstationwherethe
incidentisbeingencodedto,theprecedingproceduresstillapply.However,
immediatelythereafter,asoftcopyofthesameshouldbebroughtbytheCrime
Registrartothenearestpoliceunit/officewithinternetconnection,foruploading
intotheCIRS.

ThisMCsetsforththeguidelinesand
proceduresinthepropermanagement
andinformationaccessofdatainthe
policeblotterandthoseencodedin
CIRAS.
Since2016,CIRASwasutilizedasthe
repositoryofallcrimeincident
informationemanatingfromthe
compliantreceivedbythepoliceand
thosewritteninpoliceblotter.Such
reportsincludesinformationof
complainants,victimsandsuspects
whicharedeemedconfidential.

Disclosureofthesesinformation,mostespeciallytothosewhoarenotdirectly
involvedmayresultstoinformationmisuse.
Sincethepoliceblotterisconsideredapublicdocument,stakeholderssuchasmedia
practitionersandstudentsamongothershaveperceivedsuchasadocumentwhich
theycanfreelyaccess,view,browseandreproduce.
However,suchpracticeviolatesexistinglawsandpoliciessuchastheDataPrivacy
Actof2012andtheprovisionsstatedinthePNPFOIManual.

TheDataPrivacyActof2012prohibitsthedisclosureofanypersonalinformation

anyonewithoutthepermissionofthepersonconcerned
Inconsonance,undertheprovisionsstipulatedinthePNPFOIManual,thePNPshall
ensurethatinformationinitscustodyorunderitscontrolisdisclosedonlyas
permittedbyexistinglawsandshalllikewiseprotectpersonalinformationinits
custodyorunderitscontrolbymakingreasonablesecurityarrangementsagainst
unauthorizedaccess,leaksorprematuredisclosure

GENERALGUIDELINES
ThepoliceblotterandCIRASentriesareconsideredpropertiesofthePNPandits
officersshallbeheldresponsibleforitsconfidentiality.
PoliceblotterandCIRASentriescontainpersonalinformationofcomplainants,victims,
andsuspects.UndertheDataPrivacyActof2012,thesameshallnotbedivulgedto
anyoneunlesstheyareamongtherealparty-interestorthereisanorderfromthe
court.
AccesstothepoliceblottersandCIRASshallbeonlybelimitedtoPNPpersonnel.All
PNPpersonnelwhohaveaccessedtheinformationfromthepoliceblotter,Pink
BlotterandCIRASshallmaintaintheconfidentialityofallinformationthereinand
ensurethatsucharefreefrominformationleakage

GENERALGUIDELINES
Allinformationrequestshallbedeniedexceptthoseissuedbythedatasubjector
informationowner,orthoseissuedbythecourts.
AsprovidedbyRA9262,informationfromthePinkBlottershallnotbedivulgedto
anyoneexceptthosewhoaredirectlyinvolvedintteinvestigationofthecase.
AsprovidedbyRA6713,Sec,7,para(c),Publicofficialsandemployeesshallnotuse
ordivulge,confidentialorclassifiedinformationofficiallyknowntothembyreasonof
theirofficeandnotmadeavailabletothepubliceithertofurthertheirprivateinterest
orgiveundueadvantagetoanyone;ortoprejudicethepublicinterest
