EPIDEMIOLOGIA 02

La policitemia es un trastorno relativamente común que se presenta en el 1-5% de los neonatos. Es más común en bebés pequeños para su edad gestacional (PEG) y en bebés grandes para su edad gestacional (GEG). Los bebés nacidos a mayor altitud también tienen una mayor incidencia. Sin embargo, la mayoría de los bebés con policitemia tienen un tamaño o peso apropiado para su edad gestacional (EGA). Los bebés de madres con diabetes tienen una incidencia de policitemia del 10-30%.

Se basa en la determinación del hematocrito venoso central.
El hematocrito en las primeras dos horas de vida se encuentra elevado y se estabiliza a las doce horas de vida.
Se considera hematocrito anormal cuando es >65% después de las doce horas de vida en muestra de sangre de vena mayor) central (En sangre capilar el hematocrito es 5-15% Diagnóstico Pantoja, M. La policitemia debida a un exceso en la producción de glóbulos rojos condicionado por el incremento de la eritropoyetina fetal en respuesta a eventos hipóxicos. Manifestaciones Clinicas 05

El abanico de síntomas y signos relacionados con la policitemia en el neonato es amplio. Los síntomas más frecuentemente descritos en diferentes series de casos son: La plétora facial (dato de mayor valor diagnóstico) La letargia La mayoría de los recién nacidos con policitemia son asintomáticos y las manifestaciones clínicas son usualmente secundarias a la hiperviscosidad sanguínea. Las alteraciones que con mayor frecuencia se presentan en los recién nacidos policitemicos sintomáticos son: Ictericia Trombocitopenia Hipoglucemia Hipocalcemia Existe mayor riesgo de desarrollar una enterocolitis necrosante por la disminución del flujo sanguíneo a nivel mesentérico secundario a la hiperviscosidad sanguínea Policitemia en el recién nacido M.
