WORK-PERMIT
SYSTEM
PERMIT TO WORK (PTW)
A work permit is a document giving
permission for specific work to be carried
out subject to certain stated conditions and
with in a specified period of time
Safety work permit is a written document
authorizing person to carry out work,
warning them of the concerned dangers and
spelling out the precautions needed for
carrying out the job safely
WHY PERMIT TO WORK?
Permit specifies the conditions and
procedure for safe execution.
Basic purpose of permit system is to
prevent injuries to personnel
Maint. Personnel are not adequately
familiar with process conditions.
Operation will determine that it is safe
to perform the designated job.
Protect property from damage
Avoid fire
Ensure that all work is carried out in
safest possible manner.
REQUIRMENT OF
PERMIT TO WORK (PTW)
Permit is required for the following jobs :
Major & Minor Maint. work.
Inspection.
Construction.
Alteration.
Process Equip. cleaning
Entry into confined space.
Excavation.
Electrical job.
Height jobs etc.
WORK
PERMIT NOT REQUIRED
Routine welding / cutting and similar maintenance /
fabrication jobs carried in Central workshop
Routine sampling for the purpose of laboratory analysis.
Replacement of control card, annunciation card / lamp,
charts, and inking of charts, panel lamp etc.
Watering / gardening work relating to horticulture.
Manual sweeping with or without vacuum cleaners of
plant area etc.
Entry of stores vehicle for loading / unloading of material
/ Chemicals at safe place in the plant.

HOT WORK PERMIT
Hot work permit is required for any jobs
involving generation of sparks
Hot work : is any work such as welding /
cutting, drilling, grinding, buffing, brazing,
chipping, wall & floor breaking by spark –
producing tools etc that is capable of causing
fire condition
VESSEL/ CONFINED SPACE ENTRY
A confined space is defined as an area, which may or may
not be enclosed. It is large enough and a person can bodily
enter and perform assigned work. It is not designed for
continuous human occupancy. It has got limited or
restricted means for entry and exit.
i) It is subjected to potential hazards, viz., accumulation of
toxic / flammable gas / vapor, inert gas, oxygen deficient
(<20% v /v ), flooding of liquid, steam or any other
hazardous substance.
Some of the examples of confined space are oil tanks,
water tanks, condensate tanks, reactors, oil water sewer
pits, tanker trucks, vessels, floating roof decks, ducts, etc.
HEIGHT WORKS All types of height jobs 2 m
& above, except those jobs where working
platform with railing and Stairs /Monkey
ladder is provided.
WORK ON ELECTRICAL INSTALLATIONS
All types of electrical jobs in plants including
PCC maintenance, Transformer maintenance
& bus couplers maintenance, Electrical
equipments maintenance.
EXCAVATION JOBS
Excavation jobs in company premises
like digging, breaking of
floors/foundations etc.
LINE OPENING
The line opening jobs means opening of
any line containing or has contained
any hazardous material by removing
bolts of flanged joints, TC joints, valves
or cutting of lines.
WHO CAN SIGN A WORK
PERMIT
Personnel who are authorized to sign on
the Work Permit shall have knowledge
of the work to be performed, hazards
associated with the work and the
conditions under which the work will be
performed.
ORIGINATOR OF WORK
PERMIT
Originator is the HOD or Area Incharge of
the department in which job is to be
performed.
Originator is responsible for preparation and
handing over of the equipment/area in safe
condition to perform the work.
Authority to issue all types of work permits is
generally granted to shift incharge level
executive and above.
EXECUTING
DEPARTMENT:
Department who performs the said job.
Executing (work permit acceptor) department
representative is the engineer shift incharge from
maintenance dept. such as mechanical maintenance /
electrical / instrument /civil etc.
Executing department representative will check that
all precautions mentioned on the work permits are
taken.
He will assign the job to technicians/fitters/welders
etc. as per demand of work
He will detail all hazards associated with the job to the
workers
SAFETY DEPARTMENT
Safety department representative will
check that the all required precautions
are taken & are adequate, work location
conditions are as per mentioned on work
permit
Then he will authorize the work permit

WORK PERMIT
Surrounding area cleared of
Flammable/combustible materials
Equipment/ Vessel free of any solvents
Area cordoned off
Pressure water line/Fire extinguisher available
near workplace
Arrangement to prevent passage of
spatter/molten metal particles to surrounding
area
Proper ventilation, illumination & Emergency
Exit is available
Welding & Cutting equipments inspected
All required PPEs provided
Standby person available on site
Fire watch available for 30 min after
completion of job
PRECAUTIONS FOR
HEIGHT WORK PERMIT
Area below is cordoned off
Safety belt (Full body harness) & safety helmet is in
use.
Workers detailed about the hazards associated with
the job
Suitable ladder/scaffolding provided
Fall arrest equipments checked
Equipments/Tools are used, protected from fall from
height
Lighting, noise & weather conditions are satisfactory
PRECAUTIONS FOR
VESSEL ENTRY PERMIT
Equipment/vessel cleaned, washed & cooled to
room temperature
All lines connected to vessel are disconnected/
blinded, valves on welded lines are closed, locked
& tagged
Fuse on electrical main to agitator removed, main
switch brought to OFF position, locked & tagged
Tank/vessel made free of Toxic, flammable
gases/vapors
Oxygen content checked by Safety Dept.
Representative, found more than 19.5%
Combustible gases found nil
Safety Harness with life line provided
All required personnel protection provided
PRECAUTIONS FOR
VESSEL ENTRY PERMIT
A standby observer is deputed at man hole
for continuous monitoring & holding the life
line
Person entering in the vessel detailed about
the hazards associated with the work.
Keep fire equipment stand-by
Barricade the area
A board is displayed at manhole of “Person
working inside the vessel/Tank”
Allow minimum possible no. of persons
inside confined space

To ensure positive isolation & maintain
it all through
Ensure adequate ventilation all through
Display copy of entry permit near entry
manhole
ELECTRICAL ISOLATION
Before issuing a Work Permit, it is
essential to ensure that the equipment /
facility to be worked on is electrically
safe and electrical power is isolated to
the extent necessary for the safe conduct
of the authorized work.
Issuing, Executing authorities and
Electrical Maintenance shall jointly
decide the extent of isolation required.
‘DO NOT OPERATE’ tag shall be used
when isolation by local switch is decided
as adequate protection.
LOTO (Lock Out Tag Out) System to be
followed.
Material last present in the line is completely
drained & flushed with water
Verified that line is depressurized
All valves properly positioned, tagged, blanked
& locked
Line is disconnected on either end/supply
source
Personnel assigned the job explained on the job
safe work method
Area cordoned off, cautionary sign board
displayed
All required PPEs are provided
Line tested for flammable vapors by Safety
Dept. representative.

safeguarded

VALIDITY OF PERMIT
Work permit will be valid only for time
& date mentioned on work permit..
A fresh permit is to be obtained for next
date
If extension required on same date,
clearance to be obtained from
Originator, Executing & Safety
department. .
SURRENDERING WORK
PERMIT
When Job is Completed the work permit
should be surrendered to Originator
department after handing over the
workplace/equipment.
DISTRIBUTION OF COPIES
OF WORK PERMIT
White Copy (Original): To be
retained with the person performing
the job
Yellow Copy: To the Safety
Department
Pink Copy: To Security Gate (For
Hot work Only)
