Objectives of Technical Audit The main objective of this consulting service is to conduct third party technical audits in t his Project : to check for the compliance of the works with the contract specfn , In order to increase quality of construction, Verify design standards, Maintain the sustainability of public development activities through the assessment of cost effectiveness, Work specification and financial reviews of project activities, Impact evaluation of the project which shall also be computed to obtain meaningful feedback for future and ongoing projects.
Scopes of Technical Audit The constructed structures with regards to the conformance to the specifications with respect especially proposed dimensions and construction quality as stipulated in the contract documents and relevant Guide lines and Norms . Evaluation of work completed with regards to the work plan and schedule, Quality, Cost, Claims and Litigation. Quality Assurance provisions made by the contractor, supervision arrangement, availability and quality of tools and equipment, insurance, safety and timely payment to the workers and sub-contractors etc. Identification & Disposition of the Non-Conformances found during site visit and investigation.

Working drawings should be clear with all necessary details as per contract documents. No detail drawings with clear dimensions and structural details for Earthquake resistant point of view. As per St. contract working drawings should be. 2.0 Providing and laying of Telia bricks flooring in different locations. Telia bricks flooring should be laid on unique pattern looking very nice and attractive. Found haphazard pattern in some places of floor looks ugly due to lack of good workmanship. The Pattern should be uniqueness As Specfn . 11 3.0 Project information notice Board at site. Contractor should erect the proper sized of project information board at site when the project started. Not found any such type of Information Board at site such a big project. Contractor should be erect board upto DLP. 4.0 As per site construction some part of the Building is reduced The Building should be constructed as per design and drawings if not revised drawings should be provided. Not found as per drawings in the portion of Northwest corner of Northern wing and similarly the openings and arches in south side of same wing. There should be revised in the changed dwgs , boq & whatever.
Cont …… 5.0 In the Provisional cost: Commissions of Performance bond It should not be included in the BOQ item in general. It found in Item no. 62 in the BOQ clearly. Generally it should be bared by contractor self. This cost should not be paid to the contractor in final payment. 6.0 Revised work Schedule of the works. Contractor should submit the revised work schedule after the extension of time period. Not submitted the revised work plans by the contractor. Negligence by contactor 7.0 Timber/Sal wood work The timber should be dry, seasoned, free from knots, cracks, shakes etc. Found longitudinally major cracks at one of the beam at northern portion of Southern wing. Which needs special treatment/replacement. Non - Compliance 8.0 Insurance of works As per Contract agreement contractor should make provision of insurance of works, third party, Workmen and Employees. Not found any insurance policies of works, third party, Workmen and Employees due to negligence of the contractor. Non - Compliance
Cont … 9.0 Quality Assurance test and Plan (QAP) Contractor should make Quality Assurance Plan and should test to assure quality. Not found any Quality Assurance test reports as well as Plans but consultant monitors quality by visual inspection only . Non - Compliance 10.0 Time over run Contractor should be completed and handed over the project on deadline as per contract. Works are about 98% completed but not submitted the further bills more than 3 rd Running bill. So the project is in delayed condition and hand over process will be becoming lengthier. PADT should take action to complete and hand over the project as soon as possible.
Time, Contract Management & Quality aspect of the Works Filed verification along Interim Payment Certificate (IPC) based on Measurement Sheet reflects the evidence of work progress. The contractor has not completed the work within the original completion date 2074/03/10 . First approved extension of Time (EoT) is upto 2074/09/09 and then upto date has no approv al for additional Eot by the client. The project is expected to complete at the end of Ashadh 2075 . The contractor has maintained the advance payment guarantee and performance bound guarantee (PBG) until getting copy of Operational Acceptance Certificate. The guarantee s has been done through ADB/Agricultural Development Bank and Nepal Bangladesh Bank . The Contractor has not done the insurance of Workers, Employee, works and third party insurance as per the contract. The price adjustment criteria is included in this contract.
Cont….. The contractor has not submitted the Quality Assurance Plan (QAP) and quality control mechanisms applied for workmanship maintained as per specification. The construction material such as aggregate, sand, flag stone, shurkhi , lime, bricks were purchased from the local supplier . Daily site diary, daily work progress was not maintained by the contractor. No any written instruction and request for inspection was found. There is not available of working drawing for site. Interim payment certificate (IPC) has been certified based on the BoQ items and payment schedule with out materials test reports for supporting documents. There is no any claim raised by the contractor. The project is in the implementation stage and hence completion certificate has not been issued.
Environmental Aspects and Social Aspects No Environmental Impact Assessment and Social audit/study has been done for this project by PADT. Therefore auditor cannot emphasize in this regards. However, when the project will be completed and handed over, the pilgrims, tourists and other visitors and public in aryaghat etc. in the Pashupati area will be of course get some benefits. Claims, Disputes & Litigation Payment to the contractor was made through Interim Payment Certificate submissions (IPCs), attached in the annex. There was three IPCs were submitted by the contractor till now. There are no any claims raised by the contractor regarding the disputes and litigation.
Non conformance Report ( NCRs) NCR 1 (Picture No.1) Item: Timber/Sal wood works. Condition expected: The timber should be dry, seasoned, free from knots, cracks, shakes etc. (Specification/Page No. 64 B. 2. Timber) Condition found: Found longitudinally major cracks at one of the main beam at northern portion of Southern wing of the Bhasmeshwor sattal . Root Causes: The used timber beam was not proper seasoned, dry and free from cracks and negligence of contractor. Proposed Disposition: Which needs special treatment for wood works/replaced the beam by 25 th Jeshtha , 2075. Recurrence Prevention Measure: The used timber beams in such place should be proper seasoned, free from knots, cracks, shakes etc. should be asper design and specification.

NCR 2 (Picture No. 2) Item: Insurance of works. Condition expected: As per Contract agreement contractor should make provision of insurance of works, third party, Workmen and Employees (Miscellaneous works BOQ SN. 62). Condition found: Not found any records of insurance policies of works, third party, Workmen and Employees. Root Causes: Due to negligence of the PADT and contractor. Proposed Disposition: The Insurance of works should be done as per bill of quantities S. no. 62 by 30 th Jeshta , 2075. Recurrence Prevention Measure: The consultant and client should have proper followed up the BOQ, Specification and Design to execute the work to the contractor as per the contract agreement.
NCR 2 & 3
NCR 3 (Picture No. 2) Item : Quality Assurance test and Plan (QAP). Condition expected: Contractor should make Quality Assurance Plan and should tests of materials to assure quality ( Asper BOQ SN. 62 and Specifications). Condition found: Not found Quality Assurance plans and any test reports to assure quality (inspected regularly visual basis only). Root Causes: There are no availability of lab facilities and no trend of lab testing in previously done projects by PADT and monitored by archeological department of GoN . Proposed Disposition: The Quality Assurance test and Plan (QAP) should be followed by the contractor and instruct to the contractor to assure about the quality of consumed materials. The PADT should be promptly decided about this concern by the 15 th Ashadh , 2075. Recurrence Prevention Measure: Quality Assurance test and Plan (QAP) should be followed by the client and contractor as per the contract agreement and PADT should be promptly decided about this concern for further projects also.

Conclusion and Recommendation Conclusion No Cost overrun, No Claims, disputes and Litigation. Regarding the Quality degradation: There are no Quality Assurance Plan (QAP) submitted by the contractor. Overall quality inspected by visual basis only. Time overrun due to: Lack of materials and labors scarcity during the time of Local election. 1 st EoT : 2074-09-09 after that No approved EoT .
Suggestion & Recommendations TA suggests that t he details/working of drawings with proper dimensions should be attached in the contracted documents . TA suggests that the design should also incorporate Earthquake guide line provided by GoN for safer construction. TA recommends that contractor should be enforced to make and implement Quality Assurance Plan (QAP) and Work Schedule timely . TA recommends that PADT is a great organization where are so many NPP projects are under construction, so It should make provision of own lab facilities for testing of materials Qualit ies .
Suggestion & Recommendations TA strongly recommends that to make provision of the Insurance of Works up to the DLP/Maintenance period . TA also recommends that to PADT to take necessary action to complete the remaining minor works and handover the project as soon as possible from the contractor. TA suggests that to PADT, implement all the works asper the design, specification and contract documents otherwise if any changes proper decision should be taken by concern authority and transferred upto the site implementation stage . TA recommends that contractor should be enforced to erect the Project information board at site up to DLP in such big type of NPP project.
