Introduction: Peptic ulcer occurs in that part of gastrointestinal tract which is exposed to gastric acid and pepsin. i.e. stomach and duodenum It occurs due to imbalance between: Aggressive factors Defensive factors *A cid *gastric mucus *Pepsin *bicarbonate secretions *Bile *prostaglandins * Helicobacter.pylori *nitric oxide

Types of peptic ulcer i n gastric ulcer : acid secretion is normal or low, it is caused due to impaired mucus and bicarbonate secretions. In duodenal ulcer: it is caused due to high acid secretion, defensive factors have no role in it. Whether production of acid is normal or low, it does contribute to ulceration as an aggressive factor.

Physiology of gastric acid secretion Gastric acid (HCL) is secreted in parietal cells in the stomach by exchange of ions in the proton pump present in it. The terminal enzyme H+K+ATPase enzyme(proton pump)which secretes H+ ions in the apical canaliculi of parietal cell can be activated by histamine, Ach, gastrin acting via their own receptor
