2. Be Vigilant Know the allergies Use double patient Identification Prior Assessment Avoid distractions while preparing medication to avoid medication errors. Always ask patient about allergies, type of reaction , and severity of reaction. Make sure the drug is administered to the correct patient by doing double check as identifying the name of the patient. All medication requires a prior assessment as review of lab values , pain ,respiratory assessment ,cardiac assessment, to ensure the patient is receiving the right medication for right problem.

3. Be diligent in dosage calculation Communicate with the patient before and after drug administration Look for the expiry date Clarify the order,if unclear Errors in medication calculation have contributed to dosage errors. Provide information about the drug before administering it to the patient. Medication may be inactive if expired and cause adverse side effect. Always ask for help or consult with the physician or charge nurse sure to resolve all doubts regarding the medication before drug administration .

Report any drug errors , adverse reaction and missed dose Be cautions while administering “High Alert Medications” Reporting allows for analysis and identification of potential errors , which can lead to improvements and sharing information for safer patient care . HIGH ALERT MEDICATION are those that are most likely to cause significant harm. The most common high alert medications are anticoagulant,narcotics,insulin and sedatives.
