Challenges of Professionalism 1. PERSONAL IDENTITY VERSUS PROFESSIONAL IDENTITY 2. PRESERVATION OF SELF-INTEGRITY 3.COMMUNICATION AND RELATIONSHIP WITH TEAM MEMBERS : 4. RELATIONSHIP WITH PATIENTS AND SOCIETY
1. PERSONAL IDENTITY VERSUS PROFESSIONAL IDENTITY
Personal identity: It is based upon certain biological entities or qualities of an individual. It is considered as the values, attitudes, memories and aspirations that are unique to an individual.
Professional identity Professional identity is defined as “the attitudes, values, knowledge, beliefs and skills shared with others within a professional group’.
According to the National League for Nursing, a nurse develops a professional identity when they internalize and consider the profession's key values, take part in personal and professional development activities, and gain experience in clinical settings as they advance in their careers. By adopting the fundamental principles of nursing and interacting with peers, teachers, and patients during their time in nursing school, nurses start to shape their professional identities.
Nurses are responsible for their lifetime learning, competency, and continual professional growth. Experience, evidence-based practice, and scientific discovery serve as the foundation for this understanding. Your training, employment, and life experiences have given you acquired knowledge, abilities, professional habits, ideals, principles, and objectives that will guide you in productive and satisfying work.
2. PRESERVATION OF SELF-INTEGRITY In simple words, integrity is doing the right thing when no one is watching and even when the choice isn't easy.
Step of Preserving Self Integrity Define personal values - These are the values that the person is not going to compromise irrespective of the consequences Analyze every choice- she is making right choices and following morally and ethically right practices. 3. Encourage Integrity- People with integrity have characteristics, like being humble, a strong sense of self, high self-esteem, and self confidence
Essential Qualities to Preserve Integrity Be true to your words Make fair decisions Practice non-judgmental approach Be polite while communicating Give due credits to others : Communicate honestly:
THREATS TO INTEGRITY IN NURSING
Deceiving Patients, Withholding Information and Falsifying the Records People define deception in different ways. For some doctors, withholding dismal facts about a terminal prognosis is not deception as they think that it will keep the patient’s morale high. On the other hand, others disagree and call this deceptive.
Withholding information Withholding information is the intentional omission of concealing the information from the patient. A good example of an actual lie in the nursing situation is when we tell a patient, or their relatives, that they’re going to be fine when we know that they will probably not survive the night. Covering up a patient’s prognosis
Lying about medical errors Lying about medical errors False entries or omissions on patient records This includes inaccurate entries about the patient’s condition, treatments or observations. Examples are reporting that a treatment or medication was given when in fact it was not; documenting times in terms of when a medication should have been given when it was actually given much later; making up values for observations which were not done; and charting for all the patients on the unit at the end of the day while not really knowing whether what is being written is accurate at the time.
Strategies to Overcome the Challenges Related to Disclosing Withholding Information Assess the patient preferences regarding communication of medical information, preferably before the information becomes available. Accept the patient's rights of receiving or withholding information from himself or the nominated representative Determine the client's ability level and reveal information to fulfill the patient’s demands and expectations while respecting the individual's preferences
Strategies to Overcome the Challenges Related to Disclosing Withholding Information Any decision to withhold medically contraindicated information must be made in consultation with the patient’s family and other members of the health care team after weighing the relative advantages and risks of postponing disclosure. Keep a close observation of the patient and provide complete disclosure when the patient can determine whether or not to access the information.
Falsifying Records Falsifying documents is the act of intentionally changing or modifying information on a document with the intention of misleading a person or an institution. The act of falsifying medical records refers to the deliberate act of altering, changing, or omitting a patient's health-related documentation with the intent to deceive or mislead. Falsification can come in numerous forms, from taking laboratory test results to changing medical history.
Destroying or hiding records or parts of records Removing a diagnostic report Inserting information without standard documentation Rewriting or destroying the record Omitting significant facts Creating records for nonexistent patients or staff Failing to record important details or events
The examples of falsifying or tempering the medical records in nursing profession Documenting medications that weren't given Noting that a patient ate a full meal when he/she didn't have it Filling in someone else’ omissions Signing someone else’ name Recording care not given Fabricating data
Reporting of Falsified Documentation Follow the policy of the institute to report falsified documentation to the immediate supervisor. Full out an incident report, which will be sent to the appropriate officer. Incident report should be a clear and chronologically arranged. A jury might consider that intentional medical record alteration, falsification, or destruction by health care professionals demonstrates malice and award punitive damages even if the tampering did not directly cause patent injury. The nurse who falsifies the record could lose her license and possibly may get punishment as per law of the state.
3.COMMUNICATION AND RELATIONSHIP WITH TEAM MEMBERS :
Strategies to improve communication among team members Active listening without interrupting the conveyed facts. Conveying an accepting attitude and empathy. Showing genuine interest by staying focused on the conversation. Use non-verbal body language of interest and concern such as leaning forward, listening carefully, maintaining eye contact, etc. Offering factual information and avoiding conveying of false assurance. Using reflection and restating the feelings and spoken words.
Strategies to improve the positive relationship with team members Open communication Effective coordination Commitment and cohesiveness
Respectful and Open Communication
Strategies for achieving respectful and open Communication in nursing. R- recognize the work or efforts of other members E- Encourage open Communication and listen others S- Speak directly with person rather than talking about them to others P – practice kindness and politeness E- Empathy and emotionality for others C- Consider dissenting opinions or disagreements of others. T- Treat everyone fairly and equally.
Respectful communication is crucial for a healthy work place atmosphere. Creating a culture that encourages good, effective, and courteous communication in the nursing profession can promote teamwork, efficiency, and cohesion. Open communication is readily sharing information between people in a transparent, honest, consistent, and dependable way.
Relationships with patient and family Ways to develop therapeutic Nurse patient relationship Great patient and give introduction. Practice effective communication by maintaining eye contact and repeating what the patient has told. Enhance the quality of patient’s experience. Ensure their basic needs are met and they are comfortable. As a medical professional, non-verbal communication skills are just as important as verbal communication for patient care. Spend a bit longer time to interact with them and their loved ones
Another way to build a great nurse and patient relationship is to make the patient laugh.
Relationship with Society A mutually beneficial relationship exists between nursing and society. Nurses help the society in the following ways: Nurses help families to learn become healthy by helping them understand the range of emotional, physical, mental and cultural experiences they encounter during health and illness. Nurses help people and their families cope with illness, deal with it, and if necessary live with it, so that other parts of their lives can continue. Nurses do more than care for individuals. They have always been at the forefront of change in health care and public health.
