Insight NiTi Heat Treatment Heat treatment of NiTi alloy is leading to Increased Flexibility and Improved Fatigue Resistance Gu, Yu, et al. "Various heat-treated nickel–titanium rotary instruments evaluated in S-shaped simulated resin canals." Journal of Dental Sciences (2016).

NiTi Heat Treatments

The transition temperature to be modified for desired properties Martensitic (soft) phase Austenitic (stiff) phase SE wire M-wire Blue wire Gold wire Austenite Martensite

The colour of the wire is a consequence of the titanium oxide thickness The wave lengths of light that is reflected from the surface depend on the thickness of the titanium oxide layer.

Metallurgy

The Gold Heat Treatment The structure is achieved by a heat-treatment process after the grinding of the file. The process is proprietary to Dentsply Sirona. The Gold heat treatment impacts the metallurgy in a favorable way to provide higher flexibility and higher resistance to cyclic fatigue. WaveOne Gold

Sodium Hypochlorite (0,5% to 6%) (to eliminate organic material) and EDTA (15%) (to eliminate mineral material and smear layer) Other irrigation solutions and devices are also advocated to allow appropriate disinfecting efficiency: Chlorhexidine, heat, ultrasonics, sonics... Root Canal Irrigation smear layer

Principals of Cleaning and Shaping ”…the apical extent of effectiveness of irrigation is a function of the depth of insertion of the needle…”

Insight Infected Canal Tissue Regardless of the file system, up to 35% of the canal walls remain untouched by instruments. Peters, O. A., K. Schonenberger, and A. Laib. Obstacles to Success Dentin Tubules

Obstacles to Success Smear Layer

Obstacles to Success Remaining Bacteria

Solutions – NaOCl Dissolves organic tissue Kills most microbes No effect on mineral components Weakens in contact with other materials, especially with low pH Toxic / aggressive effect on peri-apical tissues Na + O Cl

NaO O N O OH N HO O O ONa Solutions – EDTA Removes « Smear Layer » No bacteria killing Does not dissolve soft tissues May erode dentin with longer time exposure

Example of Irrigation & Disinfection Protocol Disinfection of tooth under rubber dam with 1% - 5.25% NaOCl Shaping under irrigation with 1% - 5.25% NaOCl After completion of shaping: irrigation with 17% EDTA for 1 min Final rinse with 1% - 5.25% NaOCl ≥ 10 ml per canal Ca(OH) 2 dressing for ≥ 7 days API / Necrosis / Retreatment Drying of canals and then obturation Irreversible Pulpitis Based on recommendations from: Baylor College of Dentistry, Dallas, Texas, USA University of Zurich Dental School, Switzerland University of Freiburg, Dental School, Germany

AH Plus ® Bioceramic sealer

AH Plus ® Bioceramic Sealer Product Description With the widespread use of rotary NiTi instruments and matched-taper gutta-percha cones, the single-cone obturation technique has become popular .  One major benefit is its simplicity .  The primary drawback of this technique is the belief that epoxy resin-sealers resorb and/or shrink allowing for the potential of bacterial migration .  Bioceramic materials do not shrink on setting, resulting in an optimal interface between the gutta-percha, sealer and dentin . Bioceramic sealers allow for the production of hydroxyapatite which prevents bacterial migration – also a significant advantage over resin sealers

AH Plus ® Bioceramic Sealer

18-04-2024 Can be made of alumina and zirconia, bioactive glass, glass ceramics, calcium silicates and calcium phosphates They are non-toxic, do not shrink, and are chemically stable within the biological environment Unique ability to form hydroxyapatite and create a chemical bond between dentin and bone structure Popularity of Bioceramics have grown significantly as a result of these benefits Bioceramics is a key trend driving root canal sealers

Single Cone obturation is a key trend driving innovative sealers With the widespread use of rotary NiTi instruments and matched-taper gutta-percha cones, the single-cone obturation technique has become popular. One major benefit is its simplicity. The primary drawback of this technique is the belief that epoxy resin-sealers resorb and/or shrink allowing for the potential of bacterial migration. Bioceramic materials do not shrink on setting, resulting in a better interface between the gutta-percha, sealer and dentin. We have a unique opportunity with Conform Fit Gutta-Percha and the sealer to improve the single cone clinical outcomes +40% more predictable shape & fit than all other GP

Concept ideas A bioceramic sealer that actually sets and is biocompatible and does not stimulate periodontal tissue (both claims tested high in PT Ultimate concept testing) 5 claims tested: Biocompatible and does not stimulate periodontal tissue Predictably sets faster (2-4 hours) than BC Sealer (4-10+ hours) Does not wash out before setting Creates the optimal environment to produce hydroxyapatite Easier to retreat and regain patency Gutta-Percha’s predictable fit along the canal more specifically in the apex A better single cone outcome with predictable fit of the cone and predictable set time of the sealer

The AH Plus ® Brand Sealer for all solutions and obturation techniques The Gold standard of sealers for all obturation techniques including warm AH Plus® Bioceramic Sealer - The perfect solution with Single Cone obturation (including all ProTaper brands)

Positioning Statement For clinicians who care about long term success and prevention of reinfection of the root canal space. AH Plus Bioceramic Sealer is ideal when combined with Dentsply Sirona’s unique Conform Fit ® Gutta-Percha cones .  AH Plus Bioceramic Sealer   is hydrophilic but does not rely on fluid to set properly because it is insoluble with excellent sealing properties and zero shrinkage.

FAQs (to be continuously updated)

FAQs Q. The ProTaper Ultimate Orifice Opener/SX has no silicon stop? A. The silicon stop for the SX is not needed to control the working length, since the SX is only used in the coronal third of the canal. Q. The Slider is very similar to ProGlider, but the silicon stop is purple ? A. Yes, the idea is to keep the standard ISO colour code sequence: purple – white – yellow – red. Q. PTG S1 and S2 have become one single shaper with white color code. There maybe a risk of confusion (since Slider is purple) as in PTG S1 is purple and S2 is white? A. The idea is to keep the standard ISO colour code sequence: purple – white – yellow – red.

FAQs Q. Finishing files F1 – F3 are gold heat treated and FX/FXL are blue heat treated. Why ? A. Initially they were all blue heat treated. However, we didn’t get satisfactory results with F1 - F3 in blue (too flexible files in these sizes and with this design). Thus we reverted to gold heat treatment that demonstrates a better unwinding resistance. Q. FX has one green ring and FXL has two yellow rings. Why? A. There’s only one green file in the sequence, so it cannot be confused. There are two yellow files in the sequence, that’s why we added two yellow rings on the FXL.

FAQs Q. Ultimate Sizes FX and FXL are very different from PTG F4 and F5 and we will need some detailed reason for this? A. FX and FXL are now considered as auxiliary sizes dedicated to treat only large and straight canals without the necessary use of the previous finishing file (F1, F2, F3) before, according to the IFU as shown below: Use the auxiliary Finishers in larger and straighter canals only, such as maxillary central incisors, some palatal or distal canals of molars, or when there is a pathologic or iatrogenic defect : Select manual auxiliary file, FINISHER FX (035.12), when working length is established and patency is confirmed. Use this stand-alone file directly and passively to follow the canal to the WL using the same technique as described above. Remove and inspect its apical flutes. When loaded with dentinal debris, the preparation is finished. If the FINISHER FX is loose at length and its apical flutes are not loaded with debris, select manual auxiliary FINISHER FXL (050.10) and use this file in the same manner described above for FINISHER FX. The preparation is finished when the apical extent of any auxiliary Finisher is loaded with debris, and the correspondingly-sized GPMC or Size Verifier fits at the WL.   For this reason, the FX has a bigger ISO size (035) and taper (12%) with standard fluted portion length (16 mm) to be able to shape such canal. When the use of the FXL is necessary after the FX, that means that the coronal two third of the canal was already shaped with the previous Fx that’s why this FXL size has only a 7 mm fluted length just to shape the apical portion.
