PULMONARY ARTERY PRESSURE MONITORING Pulmonary artery pressure monitoring is an important tool used in critical care for assessing left ventricular function, diagnosis the etiology of shock, and evaluating the patients’ response to medical interventions. P ulmonary artery monitoring is achieved by using a pulmonary artery catheter and pressure monitoring system. 7/19/2020 3

HOW MONITORING PA PRESSURE HELPS HEART FAILURE PATIENTS In many cases feeling ill is the first sign that the heart failure treatment may not be working. By monitoring pressure inside the pulmonary artery ( the blood vessel that moves blood from your heart to your lungs ), your doctor can know when to make changes to your treatment, which may reduce the need for you to go to the hospital. Clinical studies show that patients whose doctors used PA pressure sensor to manage heart failure actually had fewer hospitalizations and improved quality of life. 7/19/2020 5

HOW IT WORKS A pulmonary pressure monitoring system uses a sensor to measure the pulmonary artery pressure and heart rate. The sensor is no larger than the size of dime , with two thin loops at each end. The sensor is implanted in the pulmonary artery. It does not require any batteries or wires. You will not interfere with your daily activities or other devices such as pacemaker or defibrillator. 7/19/2020 6

HOW IT WORKS( cont ) The system comes with an electronics unit that you use to take daily measurement and send the wirelessly to your doctor. 7/19/2020 7

BENEFITS OF PA PRESSURE MONITORING Your doctor may be able to detect worsening heart failure before you feel symptom. Your chances of hospitalization may be reduced because your doctor may be able to use the information to adjust your medication before you feel sick. 7/19/2020 8

RISKS OF PA PRESSURE MONITORING Risks associated with the implantation of a PA sensor. Arrhythmias Bleeding Hematoma Infection Stroke Thrombus Myocardial infarction 7/19/2020 9

PULMONARY ARTERY CATHETER Pulmonary artery catheter are frequently inserted to asses cardiac and respiratory function as well as to guide fluid and vasoactive drug administration in the critically ill patient. Development of the balloon tipped flow directed catheter has enabled continuous direct monitoring of PA . 7/19/2020 10

INSERTION OF PULMONARY CATHETER PA monitoring must be carried out in a clinical care unit under careful examination of an experienced nursing staff. Before insertion of the catheter explain to the client. A local anesthetic will be given at the catheter insertion site. Support of the critically ill patient at this time helps promote cooperation and lessen anxiety. 7/19/2020 12

INSERTION OF PULMONARY CATHETER(Cont) All types require that a balloon tipped flow directed catheter inserted into a large vein usually the subclavian , jugular, or femoral vien , the catheter then passed into the vena cava and right atrium. In the right atrium , the balloon tip is inflated and the catheter carried rapidly by the flow of blood through the tricuspid valve into the right ventricle through the pulmonic valve ,and into branch of pulmonary artery. 7/19/2020 13

INSERTION OF PULMONARY CATHETER(Cont) When the catheter reaches a small pulmonary artery, obstructing the blood flow to distal lung tissue or backward into the right ventricle. Fluoroscopy may be used during insertion to visualize the progression of the catheter through the heart chamber to the pulmonary artery. 7/19/2020 14

INSERTION OF PULMONARY CATHETER(Cont) After the insertion of catheter is correctly positioned. The following pressure can be measured: CVP or right arterial pressure Pulmonary artery systolic and diastolic pressure, mean pulmonary pressure , and pulmonary artery wedge pressure. 7/19/2020 15
