Introduction RGU   is a special X-Ray procedure done to study the  urethra  i.e., the terminal part of urinary passage, to look for any abnormalities like stricture (narrowing) etc. It is usually done in males. When there is a poor urinary stream, straining to pass urine, dribbling of urine, injury to the genital region or when Doctor suspects an abnormality of the urethra based on other investigations like the ultrasound scanning,  uroflowmetry test results. 2

Contraindications Acute urinary tract infection Recent instrumentation 6

Equipment's Fluoroscopy unit with spot film device and tilting table. Foley catheter or penile clamp, e.g. Knutson's equipment. 7

Contrast Media HOCM or LOCM 200-300, 20 ml. Pre-warming the contrast medium will help reduce the incidence of spasm of the external sphincter. OmniPaque VisiPaque 9

Patient Preparation None 10

Preliminary Film Coned supine PA of bladder base and urethra 11

Technique The patient lies supine on the X-ray table. Using aseptic technique the penile clamp is applied or the tip of the catheter is inserted so that the balloon lies in the fossa navicularis and its balloon is inflated with 1-2 ml of water. Contrast medium is injected under fluoroscopic control. Urethral fistula or periurethral abscess is seen only on the voiding examination; reflux of contrast medium into dilated prostatic ducts is also better seen during micturition. 12

Technique 13

Filming Contrast medium is injected under fluoroscopic control and films taken in the following positions:- 30° LAO, with right leg abducted and knee flexed supine PA 30° RAO, with left leg abducted and knee flexed. 14

After Care & Complications No after care is needed. Complications (Due to the contrast medium):- Adverse reactions are rare Complications ( Due to the technique):- Acute urinary tract infection Urethral trauma Intravasation of contrast medium, especially if excessive pressure is used to overcome a stricture. 16
