8. I started the PEP Talk on May 15, 2021 during the COVID19 Pandemic. There are 3 courses in the PEP Talk. I completed the Core Course on October 9, 2021.

9. From October 23, 2021 onwards, I have been tackling Health Disorder and Health Issue Courses. May 18, 2024 marks the start of YEAR 4 of ROJoson PEP Talk. How long the PEP Talk will last will depend on our enthusiasm , discipline and perseverance .

12. What is a breast needle aspiration? How is it being done? The starting point: there is a nodule or lump or mass on the breast which may be palpable or not. Breast Needle Aspiration

13. What is a breast needle aspiration? How is it being done? A hypodermic needle attached to a syringe is inserted into the breast nodule and then aspiration is done. Breast Needle Aspiration

14. What is a breas t needle aspiration? How is it being done? The findings on and outcomes of the procedure are interpreted and analyzed to come out with a DIAGNOSIS of the breast nodule. (interpretations to be discussed later) Possible Findings: Cystic (containing fluid) Solid (not containing fluid) Mixed cystic / solid Possible DIAGNOSIS after needle aspiration with or without biopsy: BENIGN MALIGNANT Breast Needle Aspiration

15. What is a breast needle aspiration? How is it being done? As mentioned earlier: The starting point: there is a nodule or lump or mass on the breast which may be palpable OR NOT . For non-palpable breast nodule or nodule which are difficult to palpate, the ultrasound machine can be used to assist in the needle aspiration . For palpable distinct breast nodule, needle aspiration may be done without the use of ultrasound. Breast Needle Aspiration

16. What is a breast needle aspiration? How is it being done? Palpable breast nodule needle aspiration can be done in the clinic, with the patient lying down, and the procedure proper of aspiration can be completed within 1-2 minutes. Breast Needle Aspiration

17. What is a breast needle aspiration? How is it being done? Specimen obtained from a breast nodule, especially, if it is solid, is submitted for a biopsy study (microscopic study by a pathologist). Note: if the breast nodule turns out to be a macrocyst after aspiration, there is NO need to submit the fluid for biopsy. Breast Needle Aspiration

18. What are the potential side effects or complications? INVARIABLY, PAIN from the needle puncture as it is inserted into the breast nodule (pain is minimal with same level as that in blood extraction with no need for anesthesia and pain killer drugs after) Breast Needle Aspiration

19. What are the potential side effects or complications? POSSIBLE side effects or complications but uncommon: Bruising – hematoma (usually spontaneously resolved ) Infection (rare – no need for antibiotics) Needle tract implantation of breast cancer Breast Needle Aspiration

20. What are its indications? Starting point for considering needle aspiration: BREAST NODULE!!! in which malignancy is a possibility and one has to know whether it is benign or malignant as there is marked difference in treatment . Breast Needle Aspiration

21. What are its indications? FOREMOST INDICATION: A BREAST NODULE in which the considerations are either a malignancy or a non-malignancy is present. Treatment is usually markedly different for malignant and non-malignant nodule. There is a need to be definite in the diagnosis prior to institution of treatment. Breast Needle Aspiration

22. What are its indications? OTHER INDICATION: A big or huge MACROCYST in which needle aspiration can provide comfort to the patient (as a result of decrease in size or even disappearance of mass). A breast nodule needle aspiration can simultaneously be therapeutic if the nodule turns out to be cystic ( macrocyst ) with the cyst completely disappearing and diagnostic (to know exactly what the nodule is). 23. How does one interpret the outcomes of a breast needle aspiration? If fluid (watery, yellowish, brownish, grayish) is obtained (it is a cystic nodule) and there is NO residual mass (complete disappearance of nodule), diagnosis: MACROCYST (BENIGN) No need to submit specimen for biopsy. 24. How does one interpret the outcomes of a breast needle aspiration? If fluid ( watery, yellowish, greenish, brownish) is obtained (cystic) and there is a residual mass, diagnosis is the nodule is a complex mass – mixed cystic /solid – not outrightly benign or malignant). Specimen is obtained from the solid element of the nodule and submitted for biopsy. Breast Needle Aspiration

25. How does one interpret the outcomes of a breast needle aspiration? If NO fluid (watery, yellowish or brownish) is obtained, nodule is solid, cells are obtained, smeared on a glass slide, to be submitted for biopsy. May be benign or cancer. Note: at times, with experience, based on appearance of cells on the smear, I can tell the diagnosis (whether benign or malignant). However, I still wait for the pathologist’s report for the final diagnosis. Breast Needle Aspiration
