ROJoson PEP Talk I have a Patient Empowerment Program in which I like to empower the lay people or patients to take control in the management of their health.
I started the PEP Talk on May 15, 2021 during the COVID19 Pandemic. There are 3 courses in the PEP Talk. I completed the Core Course on October 9, 2021.
From October 23, 2021 onwards, I have been tackling Health Disorder and Health Issue Courses. May 18, 2024 marks the start of YEAR 4 of ROJoson PEP Talk. How long the PEP Talk will last will depend on our enthusiasm , discipline and perseverance .

MAR: The Disappearing Breast Mass GIST of MAR A 41-year-old Filipina with history of fibrocystic changes and macrocysts of the breast who developed a “big” solid non-tender dominant mass in which a breast cancer was suspected and created an alarm to all concerned. What is a medical anecdotal report or MAR? MAR is operationally defined by ROJoson as a brief written report on an actual medical event that involves an actual patient. The medical observation must have an impact on the reporter in terms of insight gained and which the reporter thinks is worth sharing with other people, usually for learning purpose. NARRATION I had a patient, MDJC, 47-year-old female who on June 25, 2024, consulted for a right breast mass noted 2 weeks prior. On physical exam, there was a 4.5 to 5 cm dominant solid firm right breast mass at the right upper outer quadrant, with ill-defined border, non-tender with no associated palpable right armpit mass or lymph node. NARRATION She has seen me before for her breast concerns. ( 2018, -2020 ) In 2018, she had a 2.5-cm macrocyst on the outer upper quadrant of the left breast which I aspirated 6 cc of serous fluid with resultant disappearance of the dominant mass. NARRATION In 2020, she consulted with a 1 to 1.5-cm dominant mass at the outer upper quadrant of the right breast. My diagnosis was another macrocyst at the right breast this time. This was supported by a cyst finding on ultrasound. The said macrocyst disappeared thereafter according to her as we did not a check-up / follow-up during the COVID19 pandemic. NARRATION June 25, 2024 was her first consult with me after the COVID19 pandemic. I was alarmed and worried about the relatively big breast mass this time and it felt solid, not evidently cystic on palpation. With the background history of macrocysts in 2018 and 2020 and she claimed the mass had decreased in size 4 to 5 days before she saw me on June 25, 2024, my primary clinical diagnosis was still a macrocyst and my secondary diagnosis was a breast cancer. NARRATION She agreed to a needle evaluation with aspiration biopsy right after my clinical evaluation. The gross evaluation finding showed solid mass. It was not cystic to indicate a macrocyst . NARRATION A week after, the biopsy result showed “few benign ductal cells.” Somehow, there was a sigh of relief, at least temporarily, because there was no report of malignant cells seen. Med Anecdotal Report - The Disappearing Breast Mass
NARRATION However, I had an ultrasound of the breasts done right after the needle evaluation procedure. The ultrasound result showed BI-RADS 4B – moderate suspicion for malignancy as the diagnosis for a 2.2 x1.1 x 1.8 cm solid fairly circumscribed lobulated hypoechoic mass, right breast at 10 o’clock. NARRATION However, I had an ultrasound of the breasts done right after the needle evaluation procedure. NARRATION On follow-up on July 2, 2024, about a week after consult on June 25, 2024, on physical examination, instead of the 4.5 to 5 cm single mass previously noted, there was now noted two distinct masses, one was about 1.5 cm and the adjacent one, 2 to 2.5 cm. Obviously, there was decrease in size of the mass palpated on June 25, 2024. With these findings, my primary clinical diagnosis was benign, most likely fibrocystic changes. NARRATION On July 16, 2024, the previously noted two right breast masses on the outer upper quadrant on July 2, 2024 had further decreased in size to the point that there was practically no dominant mass, just a little prominence or thickening. NARRATION Wanting to clear-up the suspicion of breast cancer on ultrasound, the patient had a follow-up ultrasound on July 30, 2024, two weeks after. The result showed regression of the suspicious 2.2 cm mass to 1.5 cm. Thus, BI-RADS 3 (probably benign) was given by the ultrasonographer . MAR: The Disappearing Breast Mass Reflection and learning points and issues: What kind of breast masses can usually disappear on their own ? Usually, breast masses secondary to hormonal changes can disappear on their own without treatment at all. FIBROCYSTIC CHANGES, MACROCYSTS, and ADENOSIS are breast masses secondary to hormonal changes that can disappear on their own without any treatment at all. Med Anecdotal Report - The Disappearing Breast Mass Fibrocystic changes (not disease) – presence of fibrosis (thickening) and cysts (usually microcysts – less than 1 cm). Macrocysts – big cysts – palpable – usually more than one cm – part of FIBROCYSTIC CHANGES. Adenosis – enlargement of the lobules (glandular elements) of the breast FIBROCYSTIC CHANGES most common changing findings on the breasts due to hormonal changes.

Usually, breast masses that spontaneously disappear may recur or reappear again as long as the inciting cause is still present, such as hormonal changes in the body, which are constantly present throughout life. Can it be a cancer appearing this time? If it recurs, usually it has the same diagnosis or category of diagnosis – hormonally-related cause. However, a cancer can occur also. Do not just wait for a spontaneous disappearance as it may not happen especially if the breast mass is something that may not go away such as breast cancer.
