I started the PEP Talk on May 15, 2021 during the COVID19 Pandemic. There are 3 courses in the PEP Talk. I completed the Core Course on October 9, 2021.
From October 23, 2021 onwards, I have been tackling Health Disorder and Health Issue Courses. May 18, 2024 marks the start of YEAR 4 of ROJoson PEP Talk. How long the PEP Talk will last will depend on our enthusiasm , discipline and perseverance .

The Patient Story A 30-year-old Filipina consulted me in February 2025 for a right armpit mass noted in December 2024. She previously consulted a primary care physician for the same concern and was advised to have an ultrasound of the breasts. When she consulted me for the first time, she brought with her the ultrasound report ordered by her primary care physician. Patient Story – TLT: Armpit Mass in a Female – How to Manage
The Patient Story When I examined her (her right armpit initially, then her breasts and then the left armpit), I had the following findings: 2-cm firm solid non-tender mass in the right armpit, superficial, just below the skin where the armpit hair roots are located, with no redness on the skin. No dominant mass on the breasts No dominant mass on the left armpit My primary clinical diagnosis before looking at the ultrasound she brought with her was a mass related to her armpit hair roots, most likely inflammatory. I don’t think it was an armpit lymph node as it was superficial and not deep (lymph nodes are deeper in location). Patient Story – TLT: Armpit Mass in a Female – How to Manage

Patient Story – TLT: Armpit Mass in a Female – How to Manage BIRADS 4A: Low suspicion for breast cancer. Outrightly, I disagreed with the diagnosis of the radiologist (suspicion for breast cancer).
The Patient Story I asked the patient to have a repeat ultrasound of the breast in another institution. Patient Story – TLT: Armpit Mass in a Female – How to Manage
Patient Story – TLT: Armpit Mass in a Female – How to Manage Repeat ultrasound showed BIRADS 2: Benign Finding. For me, good enough but it should be BIRADS 1: Negative Finding on the breasts
Patient Story – TLT: Armpit Mass in a Female – How to Manage The real finding(s): subcutaneous masses in the armpits with inflammation on the right armpit mass. Nothing to do with the breast. I WAS CORRECT WITH MY DIAGNOSIS.
The Patient Story What did I do thereafter? I advised the patient accordingly after the repeat ultrasound. The CORRECT DIAGNOSIS. Not to be afraid of the suspicion of breast cancer. Watchful waiting in treatment. Wash armpits frequently with soap and water. Always keep clean and dry. Follow-up / check-up with me. Mass Other terms: bukol , lump, tumor, “dominant mass” A lesion which occupies a space in the human body where it is usually not supposed to be there; it is a 3-dimensional lesion with length x width x height. (A lesion is any unusual or abnormal change in the body tissues or organs.) A mass should be differentiated from enlargement, swelling and prominence which are not 3-dimensional. “Dominant mass” – a common medical term to connote a lump, tumor or bukol – one that stands out from the rest of the normal tissues.
