वमिते वर्धते वह्निः शमं दोषा व्रजन्ति हि|| वमितं लङ्घयेत् सम्यग्जीर्णलिङ्गान्यलक्षयन्| तानि दृष्ट्वा तु पेयादिक्रमं कुर्यान्न लङ्घनम्|| (Ch.Si.6/22-23) The one who has been purified by  vamana  will have augmented  agni , leading to the alleviation of  dosha . He need to undergo fasting till the signs of proper digestion of drugs for  vamana  is seen. For one having those signs,  peyadi kramam  (regulatory diet regimen) has to be initiated. Tarpanadi samsarjana krama कफपित्ते विशुद्धेऽल्पं मद्यपे वातपैत्तिके| तर्पणादिक्रमं कुर्यात् पेयाऽभिष्यन्दयेद्धि तान्|| (Ch.Si.6/24) When there is little elimination of  kapha  and  pitta  through purification therapy, in alcoholics and in persons with predominance of  vata  and  pitta dosha , the dietary regimen called  tarpanaadi kramam  need to be started instead of  peyaadi kramam , as  peyadi  may increase  kapha  causing  abhishyandam 6 Dr Krishnanunni M S

Mechanism of Samsarjana krama Activity enhances calorie requirement Hence after the Samshodhana procedure the patients are advised to do rest and to minimize activity. Carbohydrates and fats are energy giving, so are required first B ut in lesser amount during Samsarjana Krama as reduced activities are performed by the patient Milk, egg and flesh proteins are rich is essential Amino acids. TO START WITH The Shuka Dhanya are given and that too in the form of Peya (liquid), since liquid requires less HCl compared to solid food Vilepi is semisolid requires more digestive enzymes, but both are Shuka Dhanya (cereals) Yusha ( Shimbi dhanya ) first in the form of simple without spices and the fats is added to stimulate bile secretions. Since now both the enzymes for digesting carbohydrates and fats is secreted and also the mucous is somewhat repaired. 26 Dr Krishnanunni M S

After Shuka Dhanya , Shimbi Dhanya is given. What could be the reason ??? Lastly Mamsarasa is given (protein rich) which is heavy to digest needs more enzymes and HCl to denature and also enzymes are secreted. Again the sequence for Samsarjana Krama is first Akrita and then Krita . Krita means with Saindhava , Katu Dravya and Sneha . Sneha is used to increase the bile secretion in order to digest the fats or triglycerides A ll the 3 components of food are supplied (Carbohydrates, proteins and fats) Also all the digestive e nzymes needed to digest the 3 constituents here in gradually increased in the body. 31 CONCLUSION: The standardized serving size of Samsarjana Krama is one of the unique methods of serving the diet, which maintains the fasting Blood Sugar (FBS) and Random Blood Sugar (RBS) levels higher than the regular serving group. This method reduces subjective symptoms like headache, cravings for food, giddiness and lethargy, so this standardized dose of Samsarjana Krama based on body frame and Agni can be adopted in post- Vamana/ Virechana procedures. 32 CONCLUSION: The case mentioned above was diagnosed with Vyadhi Sankara in terms of Ayurveda, patient’s condition was deteriorating, because of the Agnimandya . Discussion During the Shodhana karma the normal diet will be altered hence it is important to bring back patient to his normal diet without causing any complications. For this purpose, Acharyas have mentioned special diet pattern called samsarjana karma. Samsarjana krama is administration of food in orderly manner based on the gunas of food preparations. Using of oily food at the beginning of Samsarjana krama may cause nausea, vomiting, diarrhea, fever etc because of improper digestion. Firstly introducing carbohydrates and fibers through peya , then proteins through yusha after that fats through mamsa rasa - is the transition from simple to complex food before getting back to normal diet. 34 Dr Krishnanunni M S

Conclusion Samsarjana Krama is the sequential diet regimen followed after Vamana and Virechan a Karma to enhance the Jatharagni . It normalizes the Jatharagni and Vayu , provides nutrition and helps to normalize the body tissues, which are weakened due to Samshodhana , by arranging such plans, the diet comprised of all the tastes can be served through meals ( Annakala ) and Dosha becomes normal. Now this shows that, Even if you have undergone proper shodhana karma but if you didn’t follow samsarjana krama you will again become a rogi and it may again worsen your conditions. 35 Dr Krishnanunni M S
