Scope of NBTS Breakthrough to Excellence Workshop Dr. G.D.A. Samaranayaka

History of Transfusion Medicine 1628 - English physician William Harvey discovers the circulation of blood. 1818 - James Blundell, a British obstetrician, performs the first successful transfusion 1900 - Karl Landsteiner, an Austrian physician, discovers the first three human blood groups First blood bank was established in NHSL Blood transfusion was first performed in Sri Lanka in late 1950. Widely known to the public in 1959 after the assassination of Prime Minister S.W.R.D. Bandaranaike.

Transfusion medicine - a semi-clinical discipline

Transfusion chain

Blood collection Challenges Improving 100% voluntary non-remunerated donor pool. Maintaining sufficient supply of safe blood components aging of the population Increasing prevalence of non-communicable and communicable diseases Modern technologies Appointment system for blood donation SMS reminders/appreciation Automated blood collection devices

Processing and storage Challenges Improving storage facilities Improving the access to blood and blood products Cold chain maintenance New technologies Frozen red cells Stem cell storage

Testing Grouping and cross-matching Automated, semi-automated and manual testing methodologies Electronic crossmatches - requirement in Sri Lanka Immunohaematology Column agglutination, Gel cards, solid phase assays Erythrocytes magnetized technology Molecular technologies Low throughput - polymerase chain reaction (PCR), restriction fragment length polymorphism, allele-specific PCR High throughput technologies – Microarrays, mass spectrometry TTI testing – ELISA, Chemiluminescence, NAT HLA testing

Clinical Transfusion In the field of blood banking, the main focus of attention remains in making transfusion therapy safe.

Clinical Transfusion Rational use of blood and blood products Concept of patient blood management systems (PBMS) Transfusion guidelines AABB British Society of Haematology Australasian Society of Blood Transfusion Current transfusion guidelines in Sri Lanka needs to be updated Current Sri Lankan haemovigilance guidelines should be updated and standardized

Clinical Transfusion Cellular Therapies Regenerative medicine Platelet rich plasma - high concentration of growth factors - in tissue healing and repair. Blood derived biomaterials — fibrin sealant, platelet gel Stem cell therapies in regenerative medicine – increasing in popularity in last few years. Stem cell therapies PBSC & Cord blood as sources of stem cells Cord blood unit of NBTS – current status Autologous transplant only – no allogeneic stem cell transplant programme in government sector

Education & research Local education programmes Diploma in Transfusion medicine MD in Transfusion medicine Medical students Training for MLTs, nurses and health care assistants NBTS as an international training center WHO collaborating center Asian association of transfusion medicine Research – Worldwide research for universal red cells

Regulatory aspects of Transfusion Medicine Regulatory aspects Function of NBTS in policy making and implementation Blood act Accreditation International Organization for Standardization 9000 The Joint Commission on the Accreditation of Healthcare Organizations American Association of Blood Banks
