cable power live are blocking work area.
Corrective action,
Intervention to technition for arrange line of cable to avoid potentially the worker is stumble
and blocking access.

panel door potentially struck the head of technition.

Closed of panel door to avoid the potential incident.

When observation in the Shelter area, where was found eye drill saving on asiba tube which
potential hit scaffolder when dismantle scaffolding pipe and caused incident.
Corrective action,
Inform supervisor to removed eye drill on asiba tube immediately.

stripping cable.

- Ensure technition understand with hazard of grinding activity & contor hazard.
- Perform Safe working practice (PPE requirement) and following procedure in the
workplace.
- Perform hand power tools inspection (grinding machine) before start the activity
(switch function, cable, ELCB, Rpm available, safety guard, color coding, etc)
- Use hand gloves properly as hand gloves matrix (welding gloves)
- Ensure during grinding activity that performing at table and use ragum to avoid hand
injury/incident.
- Cable management implemented to avoid the cable damage from sharp material and
potential the people tripping.
- Before storing grinding disc in box storage ensure the grinding disc has been removed
to prevent the grinding disc is break/damaged.
