SPILL MANAGEMENT

MS.SNEHA
INFECTION CONTROL NURSE

Introduction
•In a hospital, hazardous substances such as body
fluids, drugs, cleaning fluids and other chemicals are
in very close proximity to hundreds of people each
day.
•Thus in hospital spillage of blood, body fluids or
chemicals can occur at any time due to broken or
faulty equipment or human error. Any such spill
poses risk to the staff, visitors and patients who are
extremely susceptible to infection.

Type of spill in hospital setting
❑Biological spill
❑Chemical spill
❑Mercury spill
According to the size
Minor/ small –less than 10ml
Major / large –more than 10ml

Biological spill ( Blood and body fluids)
Any fluid found in, produced by, or excreted from the human
body which includes blood, urine, faces, saliva, tears, breast milk,
CSF, semen, vaginal fluids, amniotic fluids, pleural fluids,
peritoneal fluids, bile, digestive juices, vomit and pus.
In terms of standard infection control precautions practice, body
fluids are considered hazardous and should be dealt with
immediately.

Steps to proper blood spill clean up
1.Isolate the area-keep caution board .
2.Bring spill kit
3.Wear PPE ( apron, mask, goggle and gloves)
4.Place absorbent material over the spill
5.Pour freshly prepared 1 % Sodium Hypochlorite solution
6.Allow it soak. Wait for 15 to 20 min.
7.Wipe the area with the help of scoop and brush from outer to inner.
8.Discard the material into yellow cover bag.
9.Remove PPE ( gloves, goggle, apron and mask).
Note: gloves, apron –red cover, mask-yellow, goggle-cleaning with disinfect can reuse

Cont.,
10. Label the bag, spill management ( date, ward ). Send to central
BMW
11. Wash your hands.
12. Mop the area.
13. Report the spill to HIC department
14. Document the procedure.
These steps have been created based on OSHA’s
recommendations for cleaning up blood and body fluids spill.

2. Chemical spill
1.Pharmacy spill( medicine),
2.House keeping cleaning chemicals,
3.Other chemical

3. Mercury spill
Extra items required:
A.1. x ray films
B.Container ( ½ amount of water, at the time of collections)
C.Plaster/ stick tape
D.Syringe
E.Torch / flash light

Mercury spill management
1.Do not touch the mercury directly.
2.Remove any jewelry / watch
3.Wear appropriate PPE
4.Use flash light to locate mercury
5.Use x ray film to push/ gather all mercury
6.Use syringe to push the beads into container.
7.Pick up remaining beads with stick tape

CONT.,
8. Discard the use syringe, tape and used x ray films sheet into
yellow cover.
9. Remove PPE and discard as per policy
10. Label the container and send to bio medical engineer
department
11. Hand wash.
12. Inform to HIC department
13. Document the procedure

Six things you should NEVER do
1.Never use a vacuum cleaner to clean up mercury, the vacuum cleaner
will vaporize the mercury and drastically exposure in the area.
2.Never use a broom to clean up mercury, it breaks up the mercury
droplets and moves them arounds, making it harder to decontaminate
the area.
3.Never pour mercury down the drain, it can lodge in the plumbing, and
contaminate the septic tank and sludge in sewage treatment plants.
4.Never wash mercury contaminated items in a washing machine;
mercury can contaminate the sewage system and washing machine.
5.Never continue wearing shoes and clothing that might have been
contaminated in the mercury spill, this increase the wearer’s exposure
and helps spread contamination.
6.Never burn shoes, clothing, fabric or anything's that has been
contaminated with mercury; this puts mercury into the atmosphere.
