Faculty of pharmacy Pharmaceutics department It a type of a tablet that dissolve, sprays or film that is used by placing the drug under the tongue, and it is absorbed and dissolved into the blood stream through the mucous membrane. it is considered to be absorbed very fast into the circulation, as it doesn’t pass through the digestive system and the area under the tongue is rich with tiny blood capillaries. Sublingual is used in such cases as : - Patient that cannot swallow drugs - Drugs that should be absorbed quickly into the circulation - Drugs that the digestion would decrease there effect. ( Lowey, H., 1999 ) What is Sublingual Tablet? - Buprenorphine HCL: treat the opioid dependence. - Isosorbide dinitrate : treat attack of angina. - Hyoscyamine sulphate : antispasmodic or anticholinergic to treat stomach problem like cramps, bowel control problem and bladder. - Ergotamine tartrate: treat migraine. - Asenapine : treat antipsychotic like schizophrenia and acute mania. - Fentanyl citrate: analgesic opioid agonist. - Orchard, timothy and sweet vernal: immunotherapy to treat symptoms of grass pollen allergic rhinitis - Zolipidem tartrate: sedative and hypnotic. Used to treat sleep problem. - Nitroglycerin: vasodilator. - Ondansetron: block action of chemical in the body trigger like nausea and vomiting that may cause by medicine to treat cancer - Polyethylene glycol 3350: polymer of film coat-tablet . Advantages • A relatively rapid onset of action can be achieved compared to the oral route, and the formulation can be removed if therapy is required to be discontinued. • Liver is bypassed and also drug is protected from degradation due to pH and digestive enzymes of the middle gastrointestinal tract • Improved patient compliance due to the elimination of associated pain with injections; administration of drugs in unconscious or incapacitated patients; convenience of administration as compared to injections or oral medications. • Low dosage gives high efficacy as hepatic first pass metabolism is avoided & reduces side effects. • The large contact surface of the oral cavity contributes to rapid and extensive drug absorption. • Due to rapidity in action these sublingual dosage forms are widely used in emergency conditions e.g. asthma . (Fusari,2005) • Rapid absorption and higher blood levels due to high vascularization of the region and therefore particularly useful for administration of anti-angina drugs. • They also present the advantage of providing fast dissolution or disintegration in the oral cavity, without the need for water or chewing . Disadvantages • Since sublingual administration of drugs interferes with eating, drinking , and talking , this route is generally considered unsuitable for prolonged administration . (Pawar et al, 2018) • Although this site is not well suited to sustained‐delivery systems. • Sublingual medication cannotbe used when a patient is Un-cooperative or unconscious. • The patient should not smoke while taking sublingual medication, because smoking causes vasoconstriction of the blood vessels. This will decrease the absorption of the medication.
