2. Survey Monkey can be used to:
• produce multiple-choice review items
and receive automatic item analysis
report
• conduct opinion polls
• easily gather data from colleagues

3. It’s really simple to use
To set up your free account, go to
surveymonkey.com and log on with
your user name and password.
Then follow the directions in the
following tutorial to help you use it.

4. To create new survey,
click here.
To retrieve an existing survey click here
Type title here

5. Next, choose background color.
Then type the first question by clicking here.

6. After you click “add question”,
you will be directed to a
dropdown box to choose your
answer format. I chose this
one.

7. Keep clicking “add question” button to finish
creating your survey. It will look like this.

8. Sending the Survey
The survey you create can be sent in
the following ways:
• mailed as a pop-up invitation
• e-mailed as a link
• attached to a web page as a link

9. To see your list of surveys click on “My Surveys”
You can see how many people
have responded by looking here.

10. To view results as they
come in, open this tab.
Results are
displayed
in graph &
percentage
format.
