HEART MURMURS Heart murmurs are produced by turbulent flow of blood across an abnormal valve, septal defect or outflow obstruction or by increased velocity of flow through a normal valve. Murmurs are described based on the following characters: Timing , Grade , Quality , Pitch , Configuration , Radiation/conduction , Best heard with diaphragm or bell , patient position , With breath held in inspiration or expiration , variation with other maneuvers and location of maximum intensity. Based on the timing of the murmur during cardic cycle, the murmurs can be classified as : Systolic murmurs. Diastolic murmurs.

SYSTOLIC MURMURS Systolic heart murmurs can be: Early systolic murmurs. Mid systolic/ Ejection systolic murmurs. Late systolic murmurs. Pansystolic murmurs.

MID SYSTOLIC MURMURS : The murmur begins in early systole following a murmur-free interval and ends in late systole followed by a murmur free interval before S2.

Causes of mid systolic murmurs: Aortic stenosis Pulmonary stenosis Hypertrophic Obstructive cardiomyopathy.

PAN SYSTOLIC MURMURS: The murmur begins with first heart sound and extend through entire systole. They are also known as holosystolic murmurs.

Causes Pan systolic murmurs: Mitral regurgitation. Tricuspid regurgitation. Ventricular septaldefect . Patent Ductus Arteriosus with Eisenmenger syndrome.

EARLY SYSTOLIC MURMURS: The murmur begins with first heart sound and extends upto middle or late systole.

Causes of early systolic murmurs: Acute severe MR Acute Severe TR Ventricular septal defect with pulmonary hypertension.
